摘要
目的 探讨高血压病 (EH)患者血浆镁浓度 (PMg)、红细胞镁含量 (EMC)和红细胞Mg2 +/Na+交换速率与血压的相关性。方法 用原子吸收光谱火焰法测定 5 3例EH患者PMg、EMC ,以及加镁负荷后红细胞在含钾或钠介质中 5h的时间 镁外流 ,并计算总镁外流最大速率 (VTmax)、非钠依赖性镁外流最大速率 (VNImax)和钠依赖性镁外流最大速率 (VNDmax)。结果 (1)EH组PMg与对照组比较 ,差异无显著性 [(86 8 2± 5 6 5 )、(86 3 7± 5 0 8) μmol/L],EMC明显低于对照组 [(2 0 7± 0 0 5 )、(2 16± 0 0 4)mmol/L ,P <0 0 5 ],而VTmax和VNDmax明显高于对照组 [分别为 (30 8 3± 19 7)、(2 73 5±16 8) μmol·L-1·h-1,(15 3 5± 17 4)、(96 8± 13 2 ) μmol·L-1·h-1,P <0 0 5 )。VNImax两组间差异无显著性 [(16 9 5± 2 1 4)、(172 3± 15 3) μmol·L-1·h-1,P >0 0 5 ]。 (2 )EH组VNDmax与EMC呈明显正相关(r=- 0 876 4,P <0 0 0 1) ,而对照组无明显相关。 (3)EH组EMC与血压呈显著正相关 (收缩压r =0 5 5 3,P =0 0 46 ;舒张压r=- 0 42 6 ,P =0 0 2 5 ) ,同时VNDmax与舒张压呈显著正相关 (r=0 4986 ,P =0 0 41)。结论 EH患者红细胞VNDmax明显增高 ,导致EMC降低 。
Objective To measure the plasma magnesium concentration (PMg), erythrocyte magnesium content (EMC) and erythrocyte Mg 2+ /Na + exchange rate in 53 patients with essential hypertension (EH). Methods PMg and EMC were measured by flame assay of atomic absorption spectrophotometry. The one hour interval of magnesium efflux for five hours was determined, by applying magnesium loaded erythrocytes in either potassium or sodium mediated solution respectively. Maximum rates of total magnesium efflux (V Tmax ), Sodium independent magnesium efflux (V NImax ) and sodium dependent magnesium efflux (V NDmax ) were calculated. Results (1) PMg in EH was not significantly different from that in the controls (868 2±56 5 vs 863 7±50 8 μmol/L), EMC in EH was lower (169 5±21 4 vs 173 2±15 3 μmol·L -1 ·h -1 , P < 0 05), V Tmax and V NDmax were higher compared with controls (308 3±19 7 vs 273 5±16 8, and 153 5±17 4 vs 96 8±13 2 μmol· L -1 · h -1 respectively, P <0 05), while no significantly difference was found in V NImax between the two groups (2) V NDmax was significantly correlated with EMC in EH group ( r =-0 874, P <0 001), but was not significantly in the control group. (3) EMC was significantly correlative with blood pressure in EH group ( r = -0 553, P =0 046 for systolic BP, and r =-0 426, P =0 025 for diastolic BP), V NDmax was correlated with diastolic pressure significantly in EH group ( r =0 498 6, P =0 041). Conclusion Increased V NDmax in EH patients, leading to low EMC, may contribute to the pathogenesis of the disease.
