体外循环期间血小板膜糖蛋白Ib的变化

摘要 目的 :观察体外循环 (CPB)期间血小板膜糖蛋白 Ib(GPIb) ,颗粒膜蛋白 140 (GMP- 140 )及血小板大小的变化。方法 :应用全血流式细胞技术检测 19例二尖瓣置换病人 CPB前后 6个时间点血小板 GPIb,GMP- 140及血小板大小的动态变化。结果 :肝素化和 CPB均导致部分血小板活化和血小板 GPIb的降低 (P <0 .0 5 )。血小板的活化和血小板 GPIb的降低呈正相关 (P <0 .0 5 ) ,而血小板的大小无明显改变 (P >0 .0 5 )。结论 :CPB期间血小板GPIb表达的降低提供了血小板功能损伤的分子学基础 ,而肝素化亦可能是参与 Objective:Our purpose was to investigate the changes of platelet glycoprotein Ib(GPIb),platelet activation marker granule membrane protein140(GMP 140), and the size of platelets during cardiopulmonary bypass(CPB). Methods:The dynamic changes of platelet GPIb,GMP 140 and the size of platelets at 6 time points during CPB in 19 patients who were managed with mitral valve replacement were detected by the whole blood flow cytometry. Results:Both heparinization and CPB induced activation of partial platelets and decrease of platelet GPIb(P< 0.05 ).The percentage of decrease in platelet GPIb correlated well with the magnitude of activation(P<0.05).The size of platelets showed no changes during CPB(P>0.05). Conclusion:The decrease of platelet adhesion receptors provides the molecular basis for the lesions of platelet function during CPB and heparinization might be one of main factors contributing to platelet dysfunction during CPB.
