并发肺部感染的急性脑卒中患者的病原菌特点和耐药性分析

Research on Pathogenic Bacteria Distribution and Resistance in acute stroke patients with pulmonary infections

目的：探讨并发肺部感染的急性脑卒中患者痰标本培养结果病原菌特点和耐药性情况。方法：收集2010年6月～2012年6月在我院神经内科住院治疗的183例并发肺部感染的急性脑卒中患者痰标本，将其送病原菌分离培养、鉴定和药物敏感试验，总结并发肺部感染的急性脑卒中患者痰标本培养结果和耐药性情况。结果：183份遥检痰标本中检出102株病原菌，总阳性率为55．74％，其中，革兰阴性菌为主，占73．53％；检出96株细菌中，四环素和青霉素均耐药，耐药率为100．00％，对万古霉素和亚胺培南西司他丁钠敏感，耐药率为0．00％，而真菌酮康唑耐药率最高，为83．33％，两性霉素最低，为33．33％。结论：急性脑卒中肺部感染患者以感染革兰阴性杆菌为主，对四环素和青霉素均耐药，而对万古霉素和亚胺培南西司他丁钠敏感，我们在药敏结果出来前先选择耐药率低的药物进行治疗。

Objective：To probe into pathogen distribution and drug sensitivity test results of sputum speeimensin acute stroke patients with pulmonary infections. Methods：Sputum specimens from acute stroke patients with pulmonary infections who were treated in the department of neurology in our hospital from June 2010 to June 2012 were collected,then them were sent to pathogen separation,identification of cultivation and the drug sensitivity test,and in the end we summarized pathogen distribution and drug sensitivity test results of sputum speeimensin acute stroke patients with pulmonary infections. Result：One hundred and two strains of bacteria were isolated from One hundred and eighty--three sputum speclmensin acute stroke patients with pulmonary infections, total positive rate was 55. 74Yo, and most of acute stroke patients with pulmonary infections infected with gram--negative bacterium（73.53 ~ ）, ninety--six strains of bacteria to tetracycline and penicillin are the resistance, the resistant rate was 100.00 %, but bacteria to vancomycin and imipenem and cilastatin sodium were sensitive, fungus to ketoconazole, was 83.33%, amphotericin was 33.33%. Conclusion ：The most of acute stroke patients with pulmonary infections infected with gram--negative bacterium, bacteria to tetracycline and penicillin are the resistance, but it was sensitive to vaneomycin and imipenem and cilastatin sodium,. We choose sensitive drug before the drug sensitivity test results results.
