长期吸入丙酸氟替卡松对哮喘儿童骨代谢的影响 被引量:10

摘要 目的探讨长期吸入丙酸氟替卡松对哮喘儿童骨代谢的影响。方法 41例支气管哮喘患儿随机分成两组,观察组20例吸入丙酸氟替卡松(FP),对照组21例吸入二丙酸倍氯米松(BDP),疗程均为1年。观察两组吸药前后骨钙素(OC)、碱性磷酸酶(ALP)、Ⅰ型胶原羧基端伸展肽(ICTP)、Ⅰ型胶原氨基端肽(PICP)及血Ca2+,P3-含量,并与20例健康组比较;观察两组身高、体重增长速率。结果治疗前观察组、对照组、健康组3组骨代谢指标含量比较差异均无统计学意义(P均>0.05);治疗后观察组、对照组骨代谢指标含量比较差异无统计学意义(P>0.05);治疗1年后两组身高生长速率、体重增长速率比较差异均无统计学意义(P均>0.05)。结论长期吸入丙酸氟替卡松防治儿童哮喘安全、有效,不影响患儿骨代谢、身高和体重增长速率。 Objective To investigate the influence of long-termed inhaled fluficasone propionate on bone metabolism in asthmatic children. Methods Forty one cases of asthmatic children were divided into treatment group with inhaling fluticasone propionate (FP), and control group with inhaling beclomethasone dipropionate (BDP). The treatment lasted for one year in both groups. The levels of serum calcium, phosphorus, osteocalcin( OC ), alkaline phosphatase( ALP), aminoterminal propeptid of type I procoUagn (ICTP) and carboxyterminal cross-linked telopeptide of type I procollagen (PICP) were observed before and after treatment,respectively,and compared with those of 20 normal cases. The growth rates of the height and weight of two groups were also observed. Results There were no significant differences in bone metabolism among treatment group, control group and normal cases before treatment ( all P 〉 0.05 ) ; no significant difference was found in bone metabolism between treatment group, control group after treatment (P 〉 0.05 ) ; after one-year treatment, the growth rates of the height and weight showed no significant differences between two groups ( P 〉 0.05 ). Conclusion Long-termed inhaled fluticasone propionate is safe and effective for the treatment of asthmatic children, which doesn't affect their growth rates of the height and weight.
