2010年我院门诊不合理处方调查分析

Unreasonable Prescriptions Investigation of Outpatient in Our Hospital in 2010

目的:评价我院门诊处方书写质量、门诊用药基本情况及存在的问题,规范医疗行为,提高医疗质量,保障医疗安全。方法:采用回顾性分析方法,随机抽取我院2010年每周1日门诊处方进行点评、统计和分析。根据卫生部《处方管理办法(试行)》、《抗菌药物临床应用指导原则》,对处方的格式、书写规范、合理用药、抗菌药物的使用情况等几个方面进行评价。结果:共抽查评价处方6 578张,处方合格率87.64%;抗菌药物处方1 862张(28.33%),注射剂处方1 131张(17.19%),不合格处方813张(12.36%)。抽查的不合格处方中不合理用药处方占34.32%。结论:我院门诊2010年度处方合格率较高,但仍有不合理情况,应进一步重视处方点评工作。加强医师和药师业务知识学习,提高业务能力,树立良好医德医风,以确保患者的医疗安全。

Objective： To evaluate the writing quality of outpatient prescriptions in our hospital,the basic situations of outpatient medication and the existing problems.To regulate medical behavior of practitioners,improve medical quality and ensure medical safety.Methods： Outpatient prescriptions of one day per week in 2010 in our hospital were randomly selected,reviewed and statistically analyzed,by using a retrospective analysis method.According to the Prescription Management Methods（trial implementation） and Guidelines for Clinical Use of Antibacterials,the prescription format,writing specifications,rational use of drugs,and the use of antibacterials were evaluated.Results： Six thousand five hundred and seventy eight prescriptions were collected.The qualified rate of prescriptions was 87.64%.There were 1,862 antibiotic prescriptions（28.33 %） and 1,131 injection prescriptions（17.19%）.813 prescriptions were unqualified（12.36 %）.Prescriptions of unreasonable use of drugs accounted for 34.32% in unqualified prescriptions.Conclusions： Prescription qualified rate of our hospital in 2010 is high,but there are still unreasonable situations.More attentions should be paid to the prescription review.Physicians and pharmacists should strengthen professional knowledge and establish a good medical ethics to ensure medical safety.
