经桡动脉路径行弓上动脉支架成形术的现状

Transradial endovascular stenting angioplasty of the supra-aortic arteries:its current situation

作为传统的动脉入路,经股动脉入路仍为目前被广为采有的弓上动脉支架成形术的最主要路径。但这种方法的穿刺点并发症以及周围血管疾病和(或)主动脉弓的解剖变异等原因造成的技术性困难是个问题,可导致患者的手术并发症发生率增高,降低患者的最大获益。近年来,经桡动脉路径的介入治疗越来越得到重视,技术也逐渐成熟。本文复习文献,对经桡动脉路径弓上动脉支架成形术的开展情况、路径选择的理由与优势、常见的并发症等作一综述。

As a traditional arterial access in interventional treatment, transfemoral approach is still a widely used route for endovascular stenting angioplasty ofthe supra- aortic arteries. However, the complications at puncturing point, the procedural difficulties of transfemoral approach caused by peripheral vascular diseases and/or aortic arch anatomical variations, etc. have been clinical issues, as they can increase the complication incidence and reduce the patient＇s benefit. In recent years, interventional management via transradial access has attracted more and more attention from the interventional radiologists and meanwhile this technique has gradually been well-developed. This article aims to make a comprehensive review about transradial access technique for endovascular stenting angioplasty of the supra-aortic arteries, focusing on its current situation, advantages and common complications.（J Intervent Radiol, 2013, 22： 078-081）
