气管食管沟病变分析

Evaluation of lesion in the tracheosophateal groove

目的总结和分析气管食管沟病变的性质、临床特征及其手术方法。方法回顾性总结46例非甲状腺性来源的气管食管沟病变的主要症状、体格检查、影像学检查、实验室检查、手术方法、术中所见及病理结果。结果淋巴结病变12例,其中鳞状细胞癌转移者11例,反应性淋巴结增生1例;食管来源者12例,其中食管憩室9例,颈段食管癌2例,颈段食管平滑肌瘤1例;甲状旁腺来源者19例,其中甲状旁腺瘤者16例,甲状旁腺癌1例,甲状旁腺囊肿2例;支气管囊肿2例,鳃裂囊肿1例。结论气管食管沟病变来源和性质多样,99mTc-sestamibi及甲状旁腺激素检查在其术前诊断中有重要意义。

OBJECTIVE To evaluate the clinical and imaging features of lesion in the tracheosophateal groove and its management. METHODS Forty-six patients with single mass in the tracheosophateal groove, in which these from thyroid were excluded, were retrospectively studied, surgical removal was carried out according to the nature and extent of the lesion, the symptoms and signs, imaging findings, surgical mangement, laboratory and pathological resurlts were studied. RESULTS The final diagnosis establised was showed as below： lymph metastasis was found in12 cases, esophageal lesion in 12 cases, 19 parathyroid tumors or cysts, and 3 congenital diseases. CONCLUSION The mass in the tracheosophateal groove varies greatly in its nature and origin. 99mTc-sestamibi and PTH examination are of importance in makinge preoperative diagnosis of the mass in the tracheosophateal groove
