摘要 目的探讨艾滋病合并脑弓形虫感染的MRI表现及特征。方法回顾性分析已确诊的艾滋病合并脑弓形虫感染的20例MRI表现。结果病灶多发者18例,病变侵犯基底节区13例,侵犯小脑半球12例,额颞顶枕叶皮髓交界16例;MRI呈长T1、长T2异常信号,其内信号不均。增强扫描,多种病灶形态共存15例,有花环状强化19例,小结节状增强14例,片状增强9例。经抗弓形虫治疗后病变吸收好转。结论艾滋病患者增强扫描见花环状强化,高度提示脑弓形虫感染,MRI可以判断临床疗效。 Objective To investigate the MRI performance and characteristics of AIDS complicated by brain Toxoplasma infection. Methods MRI performance of 20 patients diagnosed with AIDS complicated by brain Toxoplasma infection was analyzed retrospectively. Results Eighteen patients had multiple lesions, 13 patients had lesion violating basal ganglia, 12 patients had lesion violating eerebellar hemisphere and 16 patients had lesion violating frontotemporopafi- etal occipital lobe cortical and medullary junction. MRI manifested abnormal long T1 and long T2, in which the signal was uneven. Enhanced scan showed that 15 patients had multiple lesion forms, 19 patients had flower ring like en- hancement, 14 patients had small nodular like enhancement, and 9 had flaky enhancement. The lesions were absorbed and improved after anti-Toxoplasma treatment. Conclusion Flower ring like enhancement in AIDS patients highly suggests brain Toxoplasma infection and MRI can determine the clinical efficacy.
