良性卵巢囊肿行腹腔镜手术治疗患者白细胞计数、IL-6、TNF-α的变化

Changes of leucocyte count,IL-6 and TNF-α of patients with benign ovarian cyst undergone laparoscopic surgery

目的探讨良性卵巢囊肿患者行腹腔镜手术治疗免疫功能的变化及疗效。方法将61例良性卵巢囊肿患者分为对照组(26例)和治疗组(35例)。对照组行开腹手术,而治疗组行腹腔镜手术。比较两组患者术前、术后白细胞计数、IL-6、TNF-α的变化及手术切口大小、术中出血量、手术时间等。结果两组术后24 h外周血白细胞、IL-6、TNF-α均明显升高(P<0.01),但外周血白细胞、IL-6对照组较治疗组升高幅度更大(P<0.01)。治疗组患者手术切口、术中出血量明显小、少于对照组(P<0.05),但在手术时间、手术方式上差异无统计学意义。治疗组术后镇痛用药、术后住院时间、术后肛门排气时间和术后起床活动时间均少于对照组(P<0.05),但住院费用高于对照组(P<0.05)。结论腹腔镜手术治疗良性卵巢囊肿对免疫功能的影响较开腹手术弱,更利于术后恢复,疗效更佳。

Objective To investigate the changes of immunologic function and clinical efficacy of patients with benign ovarian cyst undergone laparoscopic surgery. Methods A total of 61 patients with benign ovarian cyst were divided in- to treatment group （35 patients） and control group （26 patients）. The control group was given laparotomy, and treat- ment group were given laparoscopic operation. The leucocyte count, IL-6, TNF-α and incision size, amount of bleed- ing in operation, operation time etc. of two groups were observed and compared. Results Peripheral white blood cell, IL-6 and TNF-α increased apparently after 24 h in two groups （P 〈 0.01 ）, and treatment group was higher （P 〈 0.01 ）. The incision size, amount of bleeding in operation, postoperative pain medication, postoperative hospital stays, postoperative exhaust time and postoperative activity time of treatment group were lower than those of control group（P 〈 0.05）,and the hospitalization expenses of treatment group was higher than control group （P 〈 0.05）.There were no significant difference in operation time and modus operandi between two groups（P 〉 0.05）. Conclusion The influence on immunologic function of patients with benign ovarian cyst by laparoscopic surgery is lower than that by open surgery. The clinical efficacy of patients with benign ovarian cyst by laparoscopic surgery is better.
