高龄初产妇妊娠期糖尿病早期干预效果观察

Early intervention of gestational diabetes mellitu in elderly primiparae and newborns

目的探讨早期干预高龄初产妇妊娠期糖尿病对于母体并发症以及胎儿和新生儿结局的影响。方法对筛查出的87例高龄初产妇妊娠期糖尿病孕妇,分为血糖控制满意组(45例)和血糖控制不满意组(42例),进行母体并发症以及胎儿和新生儿结局比较分析。结果妊娠糖尿病血糖控制满意组产妇妊高症、胎膜早破人数,和围生儿病死率、巨大儿发生率、新生儿病例数,与血糖控制不满意组比较差异有显著性(P值均小于<0.05)。结论对于高龄初产妇妊娠糖尿病进行早期筛查,以及采取孕期的饮食控制、适当运动和必要时使用胰岛素等早期干预措施,对于降低产妇并发症、减少围生儿的病死率、巨大儿发生率和新生儿的患病率有重要意义。

Objective To study the early intervention of gestational diabetes mellitus （ GDM ） in elderly primiparae and newborns. Methods 87 pregnant women with GDM were divided into two experimental groups. Their gestational complications and fetal and perinatal outcomes were compared and analyzed. Results The group satisfactory in blood glucose control was significantly different from the group unsatisfactory in terms of the incidences of pregnant induced hypertension, hydramnion, fetal macrosomia and perinatal mortality and neonatal morbidity （ P 〈 0.05 ）. Conclusion Various interventions such as early diagnosis of GDM, diet control, moderate exercise and insulin therapy have important significance in reducing complications of both pregnant women and infants.
