喉癌癌变过程中细胞周期调控因子的检测

摘要 目的 探讨喉癌癌变过程中CyclinE、P2 1WAFl/CIP1和P53表达的临床病理学意义。方法 用免疫组化检测 2 0例正常粘膜、4 0例不典型增生病变的和 6 0例喉癌组织中CyclinE、p2 1WAFl/CIP1和P53的表达。结果 在喉癌癌变过程中 ,CyclinE阳性表达率分别为 :5.0 % (1/ 2 0 ) ,2 0 .0 % (8/ 4 0 ) ,4 5.0 % (2 7/ 6 0 ) (P <0 .0 0 1) ;p2 1WAFl/CIP1阳性表达率分别为 :95.0 % (19/ 2 0 ) ,75.0 % (30 / 4 0 )和 6 3.3% (38/ 6 0 ) (P <0 .0 5) ;p53阳性表达率分别为 :0 ,30 .0 % (12 / 4 0 )和 6 1.7% (37/ 6 0 ) (P <0 .0 0 1)。p2 1WAFl/CIP1在高、中和低分化的喉癌中阳性表达率呈逐渐降低的趋势 (P <0 .0 5)。CyclinE和 p53阳性表达显著相关 ,p2 1WAFl/CIP1与p53阳性表达无相关性。结论 喉癌癌变过程中CyclinE和 p53阳性表达率呈逐渐升高的趋势 ,而p2 1WAFl/CIP1阳性表达率呈逐渐降低的趋势。p2 1WAFl/CIP1表达与喉癌细胞分化程度有关。细胞周期因子相互调控机制在喉癌发展中起重要作用。 Objective To investigate the clinicopathlogical significance of the CyclinE,p21 WAFI/CIPI and p53 expression in laryngeal carcinogenesis.Methods The expression of CyclinE,P21 WAFI/CIPI and p53 was examined by immunohistochemistry method in nomal epithelia(20).dysplastic epithelia(40) and laryngeal cancers(60).ResultsIn nomal epithelium,dysplastic squamous epithelum and in squamous cell carcinoma of the larynx,the positive immunostaining rate of CyclinE protein was 5.0%(1/20),20.0% (8/40) and 45.0%(27/60) respectively(P<0.001),and P21 WAFI/CIPI protein was 95.0%(19/20),75.0%(30/40) and 63.3%(38/60) respectively(P<0.05),while the rate of p53 was 0,30.0%(12/40) and 61.7%(37/60) (P<0.001) respectively.P21 WAFI/CIPI positive expression in highly,moderately and poorly differentiated carcinomas were 76.2%(16/21),65.5%(19/29) and 30.0%(3/10) respectively(P<0.05),P21 WAFI/CIP I protein expression was significantly associated with tumour cell differentiation.The positive expression of CyclinE was closely related with the positive expression of p53(P<0.05).No correlation was found between P21 WAFI/CIPI expression and p53 expression.Conclusion CyclinE and P53 can be an early molecular event in the carcinogenesis of larynx P21 WAFI/CIPI expression is associated with tumour cell differentiation.The mutually regulating mechenism of cell cycle regulators(CyclinE,P21 WAFI/CIPI and p53) plas animporlanl role in the laryngeal carcinoma.
