左旋甲状腺素在妊娠期甲状腺功能减退患者中临床应用

The clinical application of Levothyroxine in gestation hypothyroidism patients

目的探讨妊娠期甲状腺功能减低患者的治疗方法并根据妊娠结局分析其临床效果。方法选取2008～2011年间在本院就诊的妊娠期甲状腺功能减低患者100例,本文根据患者接受药物治疗与否分为无治疗组A组50例与治疗组B组50例。评价患者的血清游离三碘甲腺原氨酸(FT3)、游离甲状腺素(FT4)、超敏促甲状腺激素(TSH)以及追踪其妊娠结局,剖宫产率、妊娠期高血压发生率、新生儿评分状况。结果 A、B组FT3、FT4、TSH治疗前后有着明显的变化,B组变化明显,接近正常。A组剖宫产率78%显著高于B组54%,妊娠期高血压发病率A组12%高于B组6%,新生儿Apgar评分A组高于B组。结论妊娠合并甲状腺功能减退患者适量应用左旋甲状腺素可明显降低妊娠并发症,改善妊娠结局。

Objective To study the gestational thyroid function of patients with reduced treatment ac- cording to the pregnancy outcome and analyze its clinical effect. Methods From 2005 to 2011, in our hospital complaining of gestational thyroid function to reduce patients in 100 cases, according to the patients received drug therapy or no treatment group is divided into A group of 50 cases and the treatment group B group of 50 ca- ses. Evaluation of FT3, FT4, TSH and the patients with pregnancy outcome, and cesarean section rate, gesta- tional hypertension incidence, neonatal score condition. Results A, B group have significantly exchange in F13, FT4 ,TSH, B can be normal, cesarean section rate was 78% in group A significantly higher than group B 54% , gestafional hypertension incidence group A 12% higher than group B 6%, and neonatal Apgar score higher than group B group A. Conclusion The pregnancy merge hypothyroidism patients right amount applica- tion levothyroxine can obviously decrease the pregnancy complication, improve the pregnancy outcome
