摘要 目的系统评价DNA异倍体检测对恶性胸腔积液的诊断价值并作Meta分析。方法计算机检索CNKI、万方、维普、Medline、Embase、Cochrane Library等数据库,筛选关于DNA异倍体检测对恶性胸腔积液诊断价值的中英文文献并进行系统评价。对纳入的文献,采用QUADAS进行质量评价,MetaDis 1.4及Stata软件进行Meta分析。结果共纳入符合要求的文献19篇,合并敏感度为0.74(95%CI:0.70~0.77),合并特异度为0.94(95%CI:0.92~0.95),合并阳性似然比为9.60(95%CI:7.53~12.24),合并阴性似然比为0.29(95%CI:0.25~0.32),合并诊断优势比为32.95(95%CI:24.01~45.22),SROC曲线下面积为0.8788。结论 DNA异倍体检测对恶性胸腔积液诊断有一定价值,其诊断准确程度中等。 Objective To evaluate the accuracy of DNA aneuploid detection in the diagnosis of malignant pleural effusion by Meta--analysis. Methods Researches about DNA aneuploid detection in the diagnosis of malignant pleural effusion were retrieved in CNKI, WANFANG DATA, WEIPU DATA, Medline, Embase and Cochrane Library. QUA- DAS was used to evaluate the quality evaluation of these articles. Statistical analysis was performed by employing Meta- Disc 1.4 software and Stata. Results Nineteen studies were adopted in the analysis. The summary estimates for DNA aneuploidy detection in the diagnosis of malignant pleural effusion were: sensitivityO. 74 (95%CI, O. 70-0. 77), specificity0.94(95%CI, 0. 92-0. 95) , positive likelihood ratio 9.60(95%CI, 7.53-12.24), negative likelihood ratio 0.29(95% CI, O. 25-0.32), diagnostic odds ratio 32.95 (95% CI, 24.01-45.22), and the AUC of SROC was 0. 8788. Conclusion DNA aneuploidy detecion plays a valuable role in the diagnosis of malignant pleural effusion, and the diagnostic accuracy is moderate.
