急性狼疮性肺炎临床特征分析

Clinical analysis of acute lupus pneumonitis in 7 cases

目的了解急性狼疮性肺炎的临床特征。方法对呼吸内科1999～2010年收治的7例急性狼疮性肺炎病例的临床特征进行分析。结果 7例患者均为女性,6例有发热,7例均有气促、低氧血症等呼吸道症状,5例有面部盘状、蝶形红斑,影像学主要表现为下肺为主的实变、浸润影,7例均提示抗核抗体(ANA)阳性,6例提示抗dsDNA阳性,主要治疗方法为全身糖皮质激素应用。结论临床以急性起病、气促、多系统损害为表现的女性患者,应考虑急性狼疮性肺炎可能,应结合相关辅助检查,尽早明确诊断和治疗。

Objective To investigate pus pneumonitis which admitted from 1999 the clinical features of acute lupus pneumonitis. Methods 7 cases of acute luto 2010 in our hospital were clinically analysed. Results All of 7 patients were female, 6 patients had fever, all of 7 patients had shortness of breath, hypoxemia and other respiratory syndromes, 5 patients had facial discoid, butterfly erythema, mainly imaging performance was consolidation and infiltration in lower lung, 7 patients were antinuclear antibody （ANA） positive, 6 cases were anti-dsDNA positive. The main treatment was systemic glucocorticoid application. Conclusion Acute lupus pneumonitis should be considered when encounter a female patient with acute onset, shortness of breath, the performance of multi--system damage, combined with relevant laboratory examinations to diagnosis and accepted treatment in time.
