摘要
目的研究血清MMP-7在IPF患者中的变化,探讨其在肺纤维化患者发病中的作用。方法收集11例IPF患者血清为实验组,收集10例健康成人血清为对照组。采用酶联免疫吸附法(ELISA)测定血清MMP-7含量,并与肺功能、动脉血气分析、呼吸困难程度进行分析。结果①IPF患者肺功能轻中度组血清MMP-7浓度(41.54±10.11)ng/mL及肺功能重度组血清MMP-7浓度(61.42±10.96)ng/mL,高于正常对照组血清MMP-7浓度(26.57±15.89)ng/mL,差异有统计学意义(P<0.05)。肺功能重度组血清MMP-7浓度高于肺功能轻中度组,差异有统计学意义(P<0.05)。②IPF患者血清MMP-7水平与动脉PaO2呈负相关(rs=-0.623,P=0.041),与呼吸困难严重程度呈正相关(rs=0.684,P=0.02),而与P(A-a)O2、PaCO2及HCO3-浓度无明显相关性(P>0.05)。结论①MMP-7可能参与了IPF的发病过程;②MMP-7可作为IPF严重程度及预后的一项参考指标。
Objective To study the changes of MMP -7 in the serum of IPF patients, and further to make clear the pathogenesis of MMP -7 in IPF. Methods The concentration of serum MMP -7 from 11 IPF patients and 10 normal control were analyzed with ELISA, while the lung function, arterial blood gases and the severity of dyspnea were detected. Results ①The MMP -7 concentrations from patients with mild to moderate lung function injury were 41.54±10.11 ng/mL and the ones from patients with severe lung function injury were 61.42±10.96 ng/mL, which were higher than those(26.57±15.89) ng/mL of the normal control group, and the difference were statistically significant ( P 〈 0.05 ) ; MMP - 7 concentrations from the group of severe lung function injury of IPF were higher than those from the group of mild to moderate lung function injury, and the difference was statistically significant ( P 〈 0. 05 ). ②The MMP-7 level showed a negative correlation( rs=-0. 623 ,P = 0. 041 ) with the PaO2 and positive correlation( r, = 0. 684 ,P = 0.02) with dyspnea severity, and no significant correlation with the concentration of P( A -a)O2 ,PaCO2 and HCO3 -. Conclusion ①MMP -7 may be involved in the path-ogenesis of IPF. ②MMP -7 can be as a reference for severity and prognosis of IPF.
