小横切口取自体骨-髌腱-骨与腘绳肌腱重建前交叉韧带的疗效比较

摘要 目的:比较分析小横切口取自体骨-髌腱-骨(bone-patellar tendon-bone,B-PT-B)与自体腘绳肌腱(hamstring tendon,HT)在膝关节镜下重建前交叉韧带(anterior cruciate ligament reconstruction,ACL)的临床疗效的差异。方法:回顾分析2003年2月至2011年3月间采用小横切口取B-PT-B在膝关节镜下重建ACL30例和HT移植32例的情况,随访平均54.3个月(18~108个月)和48.6个月(12~96个月)。采用Lysholm膝关节评分和国际膝关节文献编制委员会分级评估标准(IKDC2000)评价。结果:两组间手术时间、术后患者Lysholm评分、IKDC2000功能评分的差异均无统计学意义(P>0.05)。结论:小横切口取B-PT-B和HT移植在膝关节镜下重建ACL疗效相近,操作简便,骨愈合较好,疗效确切,手术技术的改进能有效地避免术后跪地时直接压迫皮肤瘢痕引起膝前痛。
