摘要
目的探讨应用彩色室壁运动(CK)技术和定量分析软件(ICKTM)检测慢性肾脏病(CKD)早期节段左室功能异常的临床价值。方法对28例早期CKD患者和28例年龄、性别匹配的正常对照运用彩色室壁运动技术检测左室节段收缩与舒张功能,通过ICKTM对各节段收缩指数(SI30、SI60、SI100)和舒张指数(DI)进行定量分析与比较。结果 28例正常对照左室各节段收缩与舒张运动呈现同步性,各节段SI30、SI60、SI100和DI值差异无统计学意义(P>0.05)。CKD组中12例患者出现节段收缩功能减低,相应心肌节段SI30值较正常减低,与对照组比较各节段的SI30值差异有统计学意义(P<0.05),但SI60、SI100差异无统计学意义(P>0.05)。肾病组共22例患者出现节段舒张运动减低,相应心肌节段DI值明显低于正常对照,2组间各节段的DI差异有统计学意义(P<0.05)。结论应用CK和ICKTM定量分析软件能敏感地检测出早期CKD患者左室节段功能异常,为及时发现肾病患者的心脏疾患提供诊断依据。
Objective To investigate the value of color kinesis(CK) and "ICKTM Quantitative Analysis System in evaluating regional left ventricular dysfunction in patients with early chronic kidney disease (CKD). Methods Twenty - eight patients with early chronic kidney disease and twenty - eight normal controls were subject to CK and ICKTM to analyze regional left ventricular systolic and diastolic functions. Systolic index(SI30 ,SI60 ,SI100) and diastolic index (DI) were obtained and compared between the CKD group and control group. Results Systolic and diastolic synchrony of left ventricular sections were shown in the control group, all parameters were proximal and there were no significant differences between regions (P 〉 0.05). Regional systolic dysfunction were seen in twelve patients in the CKD group, the SI30 were lower in related regions than that in the control group and there were significant differences in the two groups (P 〈0.05), but the differences of SI60 and SI100 were not significant in the two groups (P 〈0.05). Regional diastolic dysfunction were seen in twenty - two patients in CKD group, the DI were significantly lower in related regions than that in the control group, and the differences in the two groups were statistically significant (P 〈 0.05). Conclusion CK and "ICKTM Quantitative Analysis System" could evaluate the regional left ventricular dysfunction sensitively, providing early parameters in diagnosis of heart disease in early CKD patients.
