摘要
目的探讨糖尿病合并脑梗死(cerebral infarction with diabetic mellitus,DMCI)患者发病后不同时间血浆基质金属蛋白酶-9(matrix metalloproteinase-9,MMP-9)及血管假性血友病因子(von Willebrand factor,vWF)水平的变化。方法收集DMCI患者40例、非糖尿病脑梗死(cerebral infarction with non-diabetic melli-tus,nDMCI)患者35例以及健康体检者35名,检测来院即刻(发病<6h,6~8h,8~12h)血浆MMP-9及血浆vWF的水平,并对DMCI及NDMCI患者发病后24h及3d时血浆MMP-9水平进行动态观察。MMP-9、vWF检测采用酶联免疫吸附法。结果 DMCI患者各时段血浆MMP-9及vWF水平均显著高于健康体检者(P<0.05),NDMCI患者除发病6~8h外MMP-9水平均显著高于健康体检者(P<0.05),其vWF水平除发病<6h外均显著高于健康体检者(P<0.05)。DMCI组发病后24h血浆MMP-9水平〔(305.20±104.81)μg/L〕与ND-MCI组比较差异有统计学意义(P<0.01)。DMCI患者发病8~12h的vWF水平〔(210.6±60.95)μg/L〕与NDMCI组比较差异有显著性意义(P<0.05)。结论 DMCI患者发病后血浆MMP-9及vWF水平较NDMCI患者高,提示MMP-9及vWF在DMCI的发生和进展中可能具有一定临床意义。
Objective To detect the plasma levels of matrix metalloproteinase-9 (MMP-9) and von Willebrand factor (vWF) at different time points after the cerebral infarction in diabetic mellitus patients (DMCI). Methods 40 DMCI patients, 35 cerebral infarction with non-diabetic mellitus (NDMCI) patients and 35 controls were collected. Peripheral blood samples were taken at the time of their hospitalization (Three time points: less than 6 h, 6-8 h, and 8-12 h after stroke ), the plasma levels of MMP-9 and vWF factor were detected by using enzyme-linked immunosorbent assay (ELISA). The MMP-9 level of DMCI and NDMCI patients at the 24 h and 3 d after stroke was also detected respectively. Results On all the detected time points, the MMP-9 and vWF levels were higher in the DMCI patients compared to the control group (P^0.05), the MMP-9 level was higher in the NDMCI patients than in the control group except the stage of 6-8 h (P〈0.05), the vWF level was higher in the NDMCI patients than in the control group except the less than 6 h stage (P〈0.05). 24 h after the stroke, the MMP-9 of DMCI patients [ (305.20 ± 104.81) μg/L] was significantly higher than that of the NDMCI (P〈0.01). At 8-12 h, the vWF of DMCI patients [ (210.6±60.95) μg/L] was also significantly higher than that of NDMCI (P〈0.05). Conclusions The plasma MMP-9 and vWF levels post stroke can be higher in DMCI patients than that in the NDMCI patients, suggesting that the expressions of MMP-9 and vWF may have some correlation with the development and progression of DMCI.
