系统性红斑狼疮患者医院感染高危因素及耐药性研究

Study on High Risk Factors and Drug Resistance of Nosocomial Infection in Patients with Systemic Lupus Erythematosus

目的探讨分析系统性红斑狼疮患者医院感染高危因素及耐药性。方法选取2009年1月至2012年5月于本院进行治疗的223例患者为研究对象,将其医院感染发生率、高危因素及耐药性情况进行统计及比较。结果 223例系统性红斑狼疮患者中共有62例发生感染,感染发生率为27.80%,感染部位以呼吸道和泌尿道为主,以革兰阴性菌感染为主,高危因素包括住院时间、受损器官数目、是否应用激素、免疫抑制剂及抗生素应用时间、白细胞计数等,P均<0.05,而耐药性方面以耐青霉素、氨苄西林及氯霉素为主。结论系统性红斑狼疮患者医院感染高危因素包含住院时间、受损器官数目、是否应用激素、免疫抑制剂及抗生素应用时间、白细胞计数等,而耐药性方面也有其特点,应给予重视。

Objective To study and analyze the high risk factors and drug resistance of nosocomial infection in patients with systemic lupus erythematosus. Methods 223 patients with systemic lupus erythematosus in our hospital from January 2009 to May 2012 were selected as research object, and the rate of nosocomial infection, high risk factors and drug resistance of all the patients were analyzed and compared. Results 62 cases of 223 patients with systemic lupus erythematosus were with nosocomial infection, the rate of nosocomial infection was 27.80%, the site of infection mainly were respiratory system and urinary system,the pathogenic bacteria was mainly Gram negative bacteria, the high risk factors included hospitalization time, number of organ damage, treatment with or without glucocorticoid and immunosuppressive agents, application time of antibiotics and white blood cell cotmt, all P 〈0.05, and the drug resistance were mainly penicillin, ampicillin and chloromycetin. Conclusions The high risk factors include hospitalization time, number of organ damage, treatment with or without glucocorticoid and immunosuppressive agents, application time of antibiotics and white blood cell count, and the drug resistance also has its own characteristics, which should be paid attention to.
