加压交锁钉与静力交锁钉治疗78例下肢长骨骨折的对照分析

Comparative Analysis between Pressure Interlocking Nail and Static Interlocking Nail in the Treatment of 78 Patients with Lower Extremity Long Bone Fractures

目的探讨加压交锁钉与静力交锁钉治疗下肢长骨骨折的临床效果。方法选取2004年至2012年在我院治疗78例长骨骨折病例进行回顾性分析,平均分成两组,对照组予以静力交锁钉治疗,观察组予以加压交锁钉治疗,比较两组治疗后的临床效果。结果对照组的总有效率为92.31%,手术时间为(98±12)min,观察组的总有效率为97.43%,手术时间为(100±10)min,两组比较无明显的差异性,P>0.05,无统计学意义;对照组在骨折移位短缩、脂肪栓塞、锁钉松动断裂、骨折延迟愈合等并发症的发生率为12.8%,观察组相对应的发生率为5.1%,两组比较有明显的差异性,P<0.05,有统计学意义。结论加压交锁钉治疗下肢长骨骨折效果确切,可降低并发症的发生率,对增加患者的生命质量大有裨益。

Objective To investigate the clinical effect of pressure interlocking nail and static interlocking nail in the treatment of lower extremity long bone fractures. Methods 78 patients with long bone fractures treated in our hospital from 2004 to 2012 were analyzed retrospectively. They were equally divided into 2 groups, the control group was taken with static interlocking nail, and the observation group was taken with pressurized interlocking nail. The clinical effects of the two groups after treatment were compared. Results The total effective rate of the control group was 92.31%, surgery time was （98 ＋ 12） min. The total effective rate of the observation group was 97.43%, the operative time was （100 ＋_ 10） min. There were no significant differences between two groups, P〉0.05, and there was not statistically significance. The control group shift in the fracture shortening, fat embolism, lock screw loosening fracture, delayed fracture healing and so the incidence of complications was 12.8%, the corresponding rate in the observation group was 5.1%, with statistically significant difference compared between two groups （P〈0.05）. Conclusions The pressure interlocking nail in the treatment of lower extremity long bone fractures is significantly effective, and it can reduce the incidence of complications, and is beneficial to improve the patients＇ quality of life.
