妇科腹腔镜手术两种备皮方法的应用比较

Clinical Exploratory Development of Two Skin Preparation Methods for Gynecologic Laparoscopic Operation

目的:探讨妇科腹腔镜手术只消毒不剃毛的可行性。方法:选择妇科腹腔镜手术病人276例,随机分为观察组和对照组各138例,对照组采用常规腹腔镜手术备皮的方法,观察组术前用0.1%碘伏消毒腹部、会阴部,术前晚清洁灌肠后沐浴时彻底清洁会阴部。比较观察组和对照组备皮时的疼痛不适感、对病人产生的负性心理影响、备皮前后的血压、脉搏改变及术后切口感染的发生率。结果:观察组采用的备皮方法病人无疼痛不适感、未对病人产生负性心理影响;而对照组病人在剔毛的过程中自述有疼痛不适感、对病人产生的负性心理影响。观察组备皮前后血压、脉搏的改变差异无统计学意义(均P>0.05);对照组备皮前后血压、脉搏的改变等差异有统计学意义(均P<0.05)。结论:妇科腹腔镜手术采用只消毒不剃毛优于常规备皮方法。

Objective To explore the feasibility of the skin preparation method only disinfecting without shaving the fur in gynecologic laparoscopic operation. Methods 276 patients treated with gynecologic laparoscope operation were chose, which were randomly divided into observational group（138 cases） and control group（138 cases）. The control group patients were used the routine skin preparation method for laparoscope operation. But the observational group patients were treated with abdomen and perineum disinfecting with 0.1% Iodophor before operation, and the patients needed to washing and clean their perineum completely after cleaning enema at the night before operation. The discomfort feelings, the negative physiological influences of the patients, blood pressures before and after the skin preparation, the pulse changes and the ratios of postoperative infection after operation were all observed and compared between two groups. Results There were significant differences of the discomfort feelings, the negative physiological influences of the patients between two groups. The differences of the blood pressures and the pulse changes before and after the skin preparation of the control group had statistic significances （all P 0.05）. While the ratios of postoperative infection after laparoscope operation of both methods, the differences of the blood pressures as well as the pulse changes before and after the skins preparation of the observation group had no statistic significance（all P 0.05）. Conclusion The skin preparation method of disinfecting without shaving furs is better than the routine method in gynecologic laparoscope operation.
