行同期双侧开胸肺叶切除术患者的围术期护理

Perioperative nursing of patients undergoing simultaneous bilateral thoracotomy for lobectomy

目的总结行同期双侧开胸肺叶切除术患者的围术期护理要点。方法回顾性分析2011年10～11月本院2例行同期双侧开胸肺叶切除患者的临床资料,并总结护理经验。结果 2例患者手术过程顺利,术后出现心律失常1例和高血压1例。2例患者均于术后14d康复出院。结论完善的术前检查,充分的术前准备,严密的术后监护,严格的呼吸道管理及有效的镇痛治疗是行同期双侧开胸肺叶切除患者术后得以顺利康复的关键。

Objective To summarize the nursing points for the patients undergoing simultaneous bilateral thoracotomy for lobectomy. Method The clinical data of 2 patients undergoing simultaneous bilateral thoracotomy for lobectomy were retrospectively analyzed for concluding the nursing strategies. Results Both of the two patients lived through the operational course. After operation, 1 was affected with arrhythmia and the other with hypertension. After operation, drain tubes were indwelled into bilateral thoracic cavities for three days. One of them had pleural effusion right after withdrawal of drain tube, which was exhausted by thoracic close drainage. Both of them were recovered and discharged on day 14 after the operation. Conclusion The key for smooth rehabilitations of patients undergoing simultaneous bilateral thoracotomy for lobectomy exists in such nursing measures as preoperative complete examinations, full preoperative preparations, close postoperative monitoring, rigid management of respiratory tracts and effective analgesia.
