某院静脉用药调配中心不合理医嘱分析

Analysis of irrational medical order in pharmacy intravenous admixture service centre of our hospital

目的研究笔者所在医院静脉用药中心所遇到的不合理医嘱,分析其原因,以促进临床合理用药,确保患者的用药安全。方法收集笔者所在医院静脉用药调配中心2011年6月1日～2012年6月30日的不合理用药医嘱并进行整理分类,参考药品说明书、《临床用药须知》《新编药物学》《临床静脉用药调配与使用指南》及其他文献等,对每一类不合理医嘱进行统计分析。结果笔者所在医院静脉用药配置医嘱存在溶剂选用、药物配伍、给药剂量、给药浓度等方面的问题。结论药师应提高审方能力,积极与医师沟通,共同提高临床合理用药水平,保障静脉用药的安全性和有效性。

Objective To promote the rational clinical use of medicines and ensure the patients＇ safety. The unreasonable doctors＇ advices from the intravenous admixture admixture services of our hospital were study and the reasons were analyzed. Methods The unreasonable doctors＇ advices from June 1, 2011 to June 30, 2012 from the PIVAS of our hospital were collected and classified, and the reasons of each category of them were statistically analyzed according to the description of the medicines, ＂Clinical Guide＂, ＂New Pharmacology Clinical Intravenous Drug Deployment and User Guide＂ and other literatures. Results The unreasonable factors of these doctors＇ advices from the PIVAS of our hospital mainly lied in solvent selection, drug compatibility, dosage and other aspects. Conclusion Pharmacists should improve the ability of auditing the prescription, and positively communicate with the physicians to improve the rationally clinical use of medicines and assure the safety and efficacy of intravenous medication.
