腹腔镜治疗原发性脾脏良性肿瘤9例分析

Analysis of laparoscopic treatment on 9 cases of patients with primary splenic benign tumor

目的探讨腹腔镜脾切除术的体位、脾蒂的离断方法。方法回顾分析9例原发性脾脏良性肿瘤手术资料,术中据手术暴露要求调整相应体位,均采用脾动脉预阻断,二级脾蒂离断法。结果本组9例手术顺利,无中转开腹病例,术中出血少、无并发症发生,患者均痊愈出院。结论良好的体位对脾脏的显露及脾脏周围韧带的离断至关重要,脾动脉的预阻断在LS中安全可行,二级脾蒂离断法安全可靠、经济实用。

Objective To explore the position and spleen from broken＇s method of laparoscopic splenectomy （LS）. Methods A retrospective analysis of 9 cases of primary splenic benign tumors LS,according to surgical exposure requirements for adjusting the corresponding position in surgery,all patients used splenic artery preliminary block and secondary spleen from broken＇s method. Results All operative were completed successfully,there was no serious cardiovascular incident happened,intraoperative less bleeding,no complications campared,all patients were recovered. Conclusion Good position is very important to the spleen and spleen revealed from broken surrounding ligament,splenic artery in advance of the block in the preliminary block in LS is safe and feasible,secondary spleen from broken method is safe and reliable,economic and practical.
