骨髓增殖性肿瘤伴缺血性脑卒中的临床分析

The Clinical Analysis of Myeloproliferative Neoplasm Manifesting as Cerebral Ischemic Stroke

目的探讨骨髓增殖性肿瘤伴缺血性脑卒中的诊断治疗方法。方法对诊治的19例骨髓增殖性肿瘤伴缺血性脑卒中患者的发病及诊治和转归进行回顾性分析。结果 19例患者均伴发缺血性脑卒中。对19例患者进行骨髓穿刺及JAK2V617F基因突变检测,6例为原发性血小板增多症,13例为真性红细胞增多症,采用急性脑卒中的常规治疗方案联合小剂量羟基脲化疗后所有患者达到血液学缓解,神经功能均获不同程度改善。结论骨髓增殖性肿瘤伴缺血性脑卒中采用急性脑卒中的常规治疗联合小剂量羟基脲化疗及阿斯匹林治疗效果较好。

Objective Analyzing the relationship between myeloproliferative neoplasm and cerebral ischemic stroke. Methods retro-analyzing clinical characters of 19 myeloproliferative neoplasm patients with cerebral ischemic stroke. Results 19 patients all have got cerebral ischemic stroke. Bone marrow biopsy was perfomed among 19 cases. 6 cases were diagnosed essential thrombocythemia; 13 cases polycythemia vera, neurological functions were acquired different degrees of improvement in those patients with stroke, after hematologic remission was achieved with low dose hydroxyurea combined with conventional treatment of acute stroke. ConcLusion There is a close relationship between myeloproliferative neoplasm and cerebral ischemic stroke, incidence of cerebral infarction was more common than cerebral hemorrhage in those patients, low dose hydroxyurea combined with the conventional treatment and aspirin is effective for acute stroke.
