耳穴贴压联合穴位注射治疗带状疱疹临床研究

Treatment of herpes zoster by ear acupoint sticking and pressing combined with acupoint-injection

目的评价耳穴贴压联合穴位注射治疗带状疱疹的疗效及安全性。方法将110例患者随机分为治疗组(55例)和对照组(55例),对照组采用常规西药治疗,治疗组在使用西药的同时给予耳穴贴压加穴位注射。耳穴贴压选神门、内分泌、皮质下、肝、胆、肺及相应病变部位所对应的耳穴,两耳交替贴穴,每日进行按压;穴位注射选足三里、内关、曲池、太冲,进针得气后注射Vit B12注射液,每次两组穴位,两组疗程均为14 d。定期记录患者皮疹、症状及止疱时间、疼痛减轻时间、止痛时间、结痂时间、皮损痊愈时间,比较两组有效率、后遗神经痛的发生率、平均住院时间、平均住院费用,同时记录不良反应。治疗前和治疗14 d后均查血常规、大小便常规、肝肾功。结果治疗组的平均疼痛减轻时间、止痛时间、皮损痊愈时间明显短于对照组(P<0.01),有效率高于对照组(P<0.01),后遗神经痛发生率低于对照组(P<0.05),住院时间、平均住院费用低于对照组(P<0.01)。治疗组患者均未出现不良反应,治疗前、治疗后查血常规、大小便常规、肝肾功均未发现异常。结论在使用西药的同时,耳穴贴压联合穴位注射治疗带状疱疹疗效显著、安全,值得临床推广应用。

Objective To evaluate the curative effects and safety of ear acupoint sticking and pressing combined with acupoint - injection on herpes zoster. Methods 110 herpes zoster patients were randomly divided into treatment group （ n = 55 ） and control group（ n = 55 ）. All the patients were treated with conventional western medicines, and meanwhile the patients in the treatment group received ear acupoint sticking and pressing combined with acupoint . injection. Auricular points selected for ear acupoint sticking and pressing were Shenmen, and the auricular points corresponding to internal secretion, subcortex, liver, gallbladder, lung and their relative diseased regions. The acupoints of two ears were alternatively stuck with cowherb seed and pressed every day. The acupoints selected for injection of VitB12 solution were Zusanli, Neiguan, Quchi and Taichong. Two pairs of acupoints were injected at one time, and the course of treatment for both groups was 14 d. The ending time of rash, symptoms and vesiculation,time of pain relief and cease, time of incrustation and healing of skin destruction were recorded. Comparison was made in effective rate, incidence of postherpetic neuralgia, average length of stay and average cost of hospitalization between the two groups, and meanwhile the adverse reactions were recorded. Before and 14 d after the treatment, all those patients received routine blood test, routine examination of urine and stool and examination of hepatic and renal functions. Results In the treatment group, the average time of pain relief and cease and the healing time of skin destruction were significantly shorter than those in the control group（ P 〈 0.01 ） ; the effective rate was significantly higher than that of the control group（ P 〈 0.01 ） ; the incidence of postherpetic neuralgia was significantly lower than that of the control group （ P 〈 0.05 ） ; the length of stay and the average cost of hospitalization were lower than those of the control group（ P 〈 0.01 ）. Among the patients in the treatment group, no adverse reaction occurred. Before and after the treatment, no abnormality was observed in the routine blood test, routine examination of urine and stool and examination of hepatic and renal functions. Conclusion With the simultaneous treatment by western medicine ,the treatment of herpes zoster by ear acupoint sticking and pressing combined with acupoint - injection is effective and safe and worthwhile to be clinically promoted and applied.
