PAS疗法对颈动脉粥样硬化患者NO水平及斑块面积、性质的影响

Effect of PAS theray on level of nitric oxide、plaque area and plaque character in patients with carotid atherosclerosis disease

①目的采用颈动脉超声测量斑块面积、判断斑块性质,检测血中一氧化氮(NO)水平,探讨普罗布考、阿司匹林、阿托伐他汀(PAS)三者联合应用对颈动脉粥样硬化斑块患者的干预作用。②方法颈动脉粥样斑块者120例作为研究对象,随机分为PAS组和对照组。分别于治疗前、治疗后3、6个月,采用颈动脉超声测量颈动脉斑块面积、性质,采用硝酸还原酶法测定NO水平。③结果 PAS组治疗前后NO水平升高,斑块面积减少;治疗前PAS组与对照组NO水平、斑块面积无差异,6个月后PAS组的NO水平高于对照组,斑块面积低于对照组,差异具有统计学意义(P<0.01);PAS组治疗后不稳定斑块明显减少,差异有统计学意义(P<0.05)。④结论 PAS疗法对于动脉粥样硬化斑块患者具有明显改善内皮功能、稳定斑块的作用,为临床推广三者联合应用作为抗动脉粥样硬化的治疗途径提供依据,且其作用优于单纯使用降脂药物。

Objective Using ultrasound to measure carotid plaque area and judge plaque character,detecting the level of on-itric oxide, in order to investigate the effect of united therapy of probucol ,aspirin and atorvastatin in the patients with carotid athero- sclerosis disease. Methods 120 patents with arotid atherosclerosis disease were chosen for our study . They were divided into PAS group and control group randomly. Before and after the treatment in the third months and the sixth month, using ultrasound to meas-ure carotid plaque area and judge plaque character. The level of serum nitric oxide was detected by nitrate reductase. Results In the PAS group the level of serum NO after the treatment was significantly higher than that of them before the treatment , the carotid plaque area after the treatment was significantly lower than that of them before the treatment. There had no statistical significance of serum NO level and plaque area before the treatment both in the PAS group and the control group, 6 months later serum serum NO level was significantly higher than that of the control subjects, the carotid plaque area in the PAS group were significantly lower than that of the control subjects. The difference between them proved to be with statistical significance （ P 〈 0.01 ）. The instability plaque were obviously decreased after the treatment in PAS group. The difference between them proved to be with statistical signifi-cance （ P 〈 0.05 ）. Conclusion The effect of united therapy of probucol ,aspirin and atorvastation in the patients ~dth carotid ath- erosclerosis disease could contribute to improvement of endothelio function and the more stabilization of the atherosclerosis plaque. It documentation that united therapy of probucol ,aspirin and atorvastation is maked use of anti - atherosclerosis in clinical therapy, the effect of united therapy is more than atorvastation.
