胃癌侵及胰头、十二指肠的根治性手术治疗

目的探究根治术在胃癌侵袭胰头以及十二指肠治疗中的应用和分析。方法选取自2005年1月～2010年1月于广东省汕尾市人民医院普外科治疗胰头及十二指肠转移性胃癌患者61例,进行回顾性分析,其中研究组37例采用胃癌根治术,对照组24例采用胃癌姑息切除术,比较两组患者生存率以及手术并发症。结果 6个月、12个月内生存率无统计学差异(P>0.05),24个月、48个月、60个月内研究组生存率明显高于对照组,存在统计学差异(P<0.05)。两组手术并发症发生率无统计学差异(P>0.05)。术后12个月内研究组存活患者血白蛋白、前蛋白、转铁蛋白含量明显高于对照组。结论胃癌根治术可以增加5年生存率以及术后生存质量,亦不增加手术并发症,值得临床推广。

Objective To explore the radical resection in gastric cancer invasion and pancreatic and duodenal treatment application and analysis. Methods Select from January 2005 to January 2010 in the Department of general surgery in our hospital treatment of pancreatic and duodenal metastatic gastric cancer 61 cases, were analyzed retrospectively, including a study group of 37 patients with radical resection of gastric cancer, 24 cases of control group with palliative resection of gastric carcinoma, compared two groups of patients survival rate and complications of operation. Results 6 months, 12 months survival rate showed no significant difference （P 〉 0.05 ）, 24 months, 48 months, 60 months study group survival rate was significantly higher than the control group, there was significant difference （P 〈 0.05）. Two groups of operation complication rate showed no significant difference （P 〉 0.05）. Within 12 months after surgery study group survival in patients with serum albumin, transferrin, before protein content was significantly higher than that of control group. Conclusion Gastric cancer radical operation can increase the 5 year survival rate and living quality of postoperative, also does not increase the operation complications, worthy of clinical application.
