端粒酶和人乳头病毒16/18在宫颈癌中的表达及其临床意义

Expression and clinical significance of HPV16/18 and hTERC in cervical cancer

目的:探讨端粒酶(hTERC)和人乳头病毒(HPV)16/18在宫颈癌中的表达及其临床诊断意义。方法:随机选择2009年3月～2012年3月,我院妇产科收治的宫颈癌确诊病例40例作为观察组。同时,随机选择在本院门诊就诊的宫颈诊断正常40例病理标本作为对照组。分别采用聚合酶链反应(PCR)技术和重复片段扩增方法银染定性法(TRAP-PCR),对所有样本宫颈组织中hTERC和HPVl6/18的感染和表达情况进行检测。利用SPSS17.0软件运用Logistic分析各组HPVl6/18和hTERC的感染和表达与宫颈病变的关联性。结果:宫颈癌组hTERC阳性率为60.0%,显著高于正常宫颈组的2.5%(P<0.01);HPV-16/18阳性率达到92.50%,显著高于正常宫颈组阳性率7.50%(P<0.01);宫颈癌易感性与hTERC、HPV16和HPV18阳性率关联关联系数分别为3.94、4.022和4.73,均达到显著水平(P<0.05)。结论:hTERC基因表达和HPV16/18感染与宫颈癌宫颈组织病变有密切的关联性,在宫颈癌发生发展中起重要作用,两者可以作为宫颈癌早期诊断的临床指标之一。

Objective： To investigate expression and clinical significance of HPV16/18 and hTERC in cervical cancer. Methods. A total of 40 cervical cancer cases admitted from March 2009 to March 2012 were selected as study group and 40 health cases as normal control group. Polymerase chain reaction （PCR） technique and repeat fragment amplification method of silver staining qualitative method （TRAP- PCR） were used to detected the infection and expression of hTERC and HPV16/18 in all cervical tissue samples. SPSS17.0 software and logistic analysis were used to analyze the association between cervicalcancer susceptibility and infection and expression of HPV16/18 and hTERC. Results. The positive rate of hTERC in cervical cancer group was 60% , significantly higher than that of the normal cervical group （2.5%） （P〈0.01）. The positive rate of HPV-16/18 reached 92.50%, significantly higher than that of the normal cervical group （7.5 %） （P〈0.01）. The correlation coefficient between the cervical cancer sus- ceptibility and the infection and expression of HPV16/18 and hTERC was 3.94, 4.02 and 4.73, respective- ly, all reached significant level （P〈0.05）. Oonclusions：The susceptibility of cervical cancer is closely re- lated to the expression of the hTERC gene and the infection of HPV16/18 in cervical cancer tissue lesions. It plays an important role in the development of clinical index of cervical cancer. cervical carcinoma and can be used as the early diagnosis
