摘要 目的:探讨血管内皮生长因子受体flt-1及KDR在系统性红斑狼疮(SLE)皮损中的表达及意义。方法:免疫组化法(SP)检测SLE患者皮损中flt-1与KDR蛋白的表达与分布特征,同时检测皮损中的微血管密度(MVD)。结果:(1)在表皮组织中flt-1与KDR在患者组与正常对照组中均有表达,但表达强度差异无统计学意义(P>0.05),正常对照组中flt-1与KDR均匀表达于表皮全层,而患者组flt-1与KDR在颗粒层表达最强,其中SLE活动期最强,稳定期次之。(2)真皮组织中flt-1与KDR主要表达在真皮乳头微血管上,且患者组真皮乳头微血管上的表达程度要明显高于正常对照组(P<0.01),活动期最强,稳定期次之。(3)患者组皮损真皮乳头血管MVD明显高于正常对照组(P<0.01),真皮乳头微血管flt-1与KDR阳性表达组的MVD明显大于阴性表达组的MVD(P<0.01)。flt-1与KDR与MVD三者之间任何两者均存在正相关关系(flt-1与KDR相关系数为0.88,MVD与flt-1相关系数为0.66,MVD与KDR相关系数为0.82,P均<0.01)。(P<0.05)。结论:SLE患者皮损中VEGF受体flt-1与KDR蛋白均高表达。真皮乳头微血管flt-1与KDR的表达与真皮乳头血管的生成存在正相关关系。 Objective: To discuss the expression of vascular endothelial growth factor (VEGF) of receptors (Flt-1 and KDR) in the lesional skin of SLE patients. Methods: Distribution characteristics of Flt-1 and KDR were detected with SP, and the microvessel density (MVD) in the lesional skin was coun- ted as well. Results:①The expression of Flt-1 and KDR was observed in lesional skin of both patients and normal controls, and the levels of both groups showed no significant difference. Flt-1 and KDR were dis-tributed evenly in all layers of epidermis in normal controls, while they were more intensively distributed in granular layer of lesional skin in SLE patients, expecially during active phase. ②The expression of Flt- 1 and KDR in dermis mainly presented in the papillary dermal microvessels, and is significantly higher than that of normal controls(P〈0.01). The highest level was recorded in active phase of SLE. ③The MVD in lesional skin was higher than that in normal controls(P〈0.01), and MVD in the papillary dermal microvessels was significantly higher in Flt-1 and KDR positive lesions than that in negative lesions. There is positive correlation between every two index of Fit-l, KDR and MVD. The related coefficient of Flt-1 and KDR is 0.88, Flt-1 and MVD is 0.66, KDR and MVD is 0.82(P all〈0.01). Oonclusion. VEGF and its receptors (Flt-1 and KDR) are over expressed in lesional skin of SLE patients, and this kind of overex- pression is positively correlated to the genesis of papillary dermal microvessels.
