卵巢上皮性肿瘤中P27和P53蛋白的表达及其相关性研究

Expression of P27 and P53 and the correlation between P27 and P53 in epithelial ovarian tumor

目的探讨P27、P53在卵巢上皮性肿瘤的发生、发展及预后中的临床价值及相关性。方法应用免疫组化S-P法检测良性上皮性肿瘤25例(良性组)、交界性肿瘤18例(交界性组)、恶性肿瘤57例(恶性组)和同期正常卵巢组织20例(对照组)P27、P53蛋白的表达。结果 (1)P27在对照组、良性组、交界性组及恶性组中阳性表达率逐渐降低,恶性组P27蛋白表达明显低于正常组、良性组,差异有统计学意义(P<0.05)。(2)卵巢上皮性癌中,P27蛋白的表达水平与年龄、病理类型及有无淋巴结转移无关,差异无统计学意义(P>0.05);与临床分期、组织分化相关,差异有统计学意义(P<0.05)。(3)P53在正常组织中未见表达,恶性组P53蛋白表达明显高于正常组、良性组和交界性组,差异有统计学意义(P<0.05)。(4)恶性组P53蛋白的表达水平在年龄、病理类型、淋巴结转移中差异无统计学意义(P>0.05);低分化组明显高于中高分化组,Ⅲ～Ⅳ期明显高于Ⅰ～Ⅱ期,差异有统计学意义(P<0.05)。(5)P27和P53表达存在负相关性(Rs=-0.397,P<0.05)。结论 P27的表达下降P53蛋白异常表达可能对卵巢上皮性肿瘤由良性向恶性的转化发展起着某种促进作用。且P27蛋白可能是作为卵巢上皮性癌恶性程度和病理分级的标志,预示预后不良。

Objective To investigate the clinical significance of P27 and P53 in the pathogenesis, development and prognosis of epithelial ovarian tumor, and to explore the correlation between P27 and P53. Methods The expressions of P27 and P53 were detected by immunohistoehemisty（SP） in 57 cases of malignant ovarian tumor（ malignancy group） , 18 cases of borderline ovarian epithelial neoplasm（ borderline group） ,25 cases of benign ovarian tumor（ benign group）and 20 cases of normal ovarian tissue samples（ control group）. Results The expression of P27 was gradually reduced in the order of control group, benign group, borderline group and malignancy group, furthermore, the expression of P27 in malignancy group was significantly lower than that in control group and benign group （ P 〈 0.05 ）. The expression of P27 in malignancy group was not related with patient＇ s age, pathological type, lymph node metastasis, and there were no significant differences （ P 〉 0.05 ）. However the expression of P27 in malignancy group was closely coirelated with clinical staging and tissue differentiation,and there were significant differences（ P 〈0.05 ）. The expression of P53 was not found in control group,however, the expression of P53 in malignancy group was significantly higher than that in control group, benign group and borderline group （ P 〈 0.05 ）. There was no significant difference in the expression of P53 in malignancy group among patient＇ s age, pathological type, lymph node metastasis （ P 〉 0.05 ） , and the expression of P53 in low differentiation group was significantly higher than that in medium and high differentiation group, which at Ⅲ -Ⅳ stage was significantly higher than that at Ⅰ-Ⅱ stage（ P 〈0.05）. The expression of P27 was negatively correlated with that of 53 （ r = - 0. 397, P 〈 0.05 ）. Conclusion The decrease of P27 expression and abnormal expression of P53 protein may play a certain promotive role in the development of epithelial ovarian cancer, furthermore,P27 protein may be the sign of malignant degree and pathological classification of epithelial ovarian tumor,which prognosticates bad prognosis.
