小剂量地佐辛抑制芬太尼诱发咳嗽反射的临床观察 被引量:7

摘要 作用于阿片μ受体的芬太尼及其衍生物静脉注射时可能诱发呛咳,与注药过快、药物浓度过高或剂量过大,患者的生理或疾病状态相关。一般认为,此效应有阿片及NMDA受体机制参与,并与C纤维感受器、快速适应性肺牵张感受器激动及组胺、枸橼酸的刺激作用相关。众所周知,激动-拮抗类阿片药与阿片μ受体激动药同用,拮抗了μ受体作用,降低了阿片药物的不良反应(包括呛咳),也会削弱总体镇痛效应,但此种拮抗作用是否有或有何种类型的剂量依赖性,以及阿片药物的各种不同作用及不良反应是否均被平行拮抗?本刊此次发表两篇所用地佐辛剂量相差颇巨的论文(50μg/kg和400μg/kg)供争鸣时参考,也借以说明此类"老"药的应用也需规范。至于制止或抑制芬太尼类药物引起呛咳的方法颇多,安全、简单、高效应/不良反应比,仍是选择的基本标准。
