摘要
目的:观察补充小剂量L-甲状腺素对早产儿暂时性低甲状腺素血症的近期疗效及安全性。方法:将符合条件的63例早产低甲状腺素血症儿分为治疗组32例和对照组31例,治疗组给予小剂量L-甲状腺素片口服,观察两组在静脉营养、光疗、住院时间、恢复出生体重、消化异常、心率及颅内异常发生率等方面差异,比较治疗组在治疗前后血生化方面改变。结果:治疗组静脉营养、光疗、恢复出生体重及住院时间比对照组明显缩短(P<0.05),消化系统异常天数减少(P<0.01),心率无明显增快(P>0.05),颅内出血及脑白质病发生率无增加(P>0.05)。治疗组在治疗前后血谷丙转氨酶、肌酐、尿素氮、前白蛋白、心肌酶及血糖无明显变化(P>0.05)。结论:早期小剂量补充甲状腺素安全有效,可以提高患儿早期生活质量,减少家长经济负担。
Objective:To observe the short-term curative effect and safety of supplementary therapy with low dose L-thyroxine for treatment of transient hypothyroxinemia in premature infants.Methods:Sixty-three premature infants with hypothyroxinemia were divided into treatment group(32 premature infants) and control group(31 premature infants);the premature infants in treatment group were treated with oral administration of low dose L-thyroxine pills;the differences of intravenous nutrition time,phototherapy time,hospitalization time,the time to return to birth weight,digestive system abnormalities,heart rate,and the incidence rates of intracranial abnormalities were observed in the two groups;the changes of blood biochemical items in treatment group before and after treatment were compared.Results:The intravenous nutrition time,phototherapy time,hospitalization time,and the time to return to birth weight in treatment group were statistically significantly shorter than those in control group(P0.05).The duration time of digestive system abnormalities in treatment group was statistically significantly shorter than that in control group(P0.01).Heart rate and the incidence rates of intracranial hemorrhage and leukoencephalopathy in treatment group didn't increase significantly(P0.05).In treatment group,the levels of alanine transaminase,creatinine,urea nitrogen,pre-albumin,myocardial enzyme,and blood glucose after treatment didn't change obviously(P0.05).Conclusion:Early supplementation of low dose thyroxine is safe and effective,which can improve the quality of life of premature infants and reduce economic burden of parents.
