双微导管技术在颅内宽颈动脉瘤栓塞治疗中的应用价值 被引量:1

摘要 目的研究双微导管技术在颅内宽颈动脉瘤临床治疗中的可行性、疗效及其应用价值。方法 13例宽颈动脉瘤患者共13枚动脉瘤,采用双微导管技术行血管内栓塞治疗。结果 13枚动脉瘤采用双微导管技术12例获得成功,技术成功率为92%,栓塞后即刻血管造影显示瘤腔闭塞、未见造影剂充填。随访2~6年,复查DSA显示动脉瘤瘤腔闭塞100%者7例(53.85%),闭塞95%以上者4例(30.77%),闭塞90%者1例(7.69%),栓塞小于80%者1例(7.69%);格拉斯预后评分,5分9例(69.23%),4分3例(23.08%),3分1例(7.69%)。结论双微导管技术是颅内宽颈动脉瘤治疗中一种可供选择、疗效确切、比较安全的的栓塞方法。 Objective To investigate the feasibility, the curative effect and the application value of double microcatheter technique in endovascular therapy of cerebral aneurysm. Methods All the patients with wide di- ameter aneurysms have 13 aneurysm altogether, and all were performed endovascular therapy with double micro- catheter technique. Results Twelve of 13 cases got successful; with the successful rate of 92.30%, Aneurysm cavity disappeared immediately in DSA image after endovascular therapy. Follow-up of 2-6 years, the cavity of 7 aneurysms were obstructed totally by DSA imaging again, 4 were occluded more than 95%, and 1 was done 90%, one was done less than 80%. By Glasgow outcome scale, 9 patients had five points, 3 patients had four points, and I patient had fthree points. Conclusion The double micro catheter technique may be an optional, effective and more safe method during the embolization of some complicated wide-necked aneurysms.
