摘要 目的探讨CD64、CD14在急性单核细胞相关的髓系白血病免疫分型鉴别诊断中的意义。方法采用CD45/SSC散点图设门多参数流式细胞术分析70例急性髓系白血病(AML)患者细胞CD64、CD14及其他相关抗原的表达情况。结果 CD64在AML上的表达情况为:M0(0.0%)、M1(0.0%)、M2(10.5%)、M3(46.2%)、M4(78.6%)、M5(100.0%),其中弱阳性、阳性表达可以出现在各型AML中,唯独强阳性表达出现在M5中(P<0.01)。CD14在AML上的表达情况为M4(57.1%)、M5(77.8%),其余均为阴性。结论 CD64对诊断AML的灵敏度优于CD14,CD14的特异度优于CD64,二者结合有助于提高AMLM4/M5型的检出率及其与其他AML亚型的鉴别诊断。 Objective To evaluate the usefulness of CD64 and CD14 by flow cytometric immunophenotyping in distinguishing acute myeloid leukemias (AMLs) with monocytic differentiation from other AML subtypes.Methods The expression of CD64,CD14 and other related antigens were analyzed in 70 prime patients with AML by CD45/SSC dot plot gated multi-parameter flow cytometry.Results The positive rates of CD64 were 0.0% in M0,0.0% in M1,10.5% in M2,46.2% in M3,78.6% in M4,100.0% in M5.CD64 expressed in varying intensities,which dimly and moderately expressed in all AML subtypes,but strongly expressed in M5 only.CD14 only expressed in M4(57.1%)and M5(77.8%).Conclusion The sensitivity of CD64 was higher than CD14 in distinguishing AML with a monocytic component from other AML subtypes,but the specificity of CD14 was higher than CD64.The combination of CD64 and CD14 could be used as a myeloid marker for distinguishing AML with a monocytic component from other AML subtypes.
