抗结核治疗前后的薄层CT表现

Pulmonary HRCT findings before and after anti-TB treatment

目的:通过描述肺结核治疗前和治疗后的薄层CT表现,评估疾病活动性和治疗的有效性。方法:回顾性分析195例患者初诊细菌学证明的肺结核;治疗前后均行薄层CT扫描。结果:微结节、结节、树芽征、实变和厚壁空洞是活动性肺结核最常见的薄层CT表现。微结节、结节和树芽征的消失、实变的吸收、厚壁空洞的缩小以及转化为薄壁空洞、胸腔积液的消退以及纤维化改变的存在表达了治疗的有效性。结论:薄层CT可以从陈旧的纤维化病变中鉴别出新的结核活动性病变,有助于判定肺结核的活动性和评估抗结核药物的疗效。

Objective：To describe the thin-secting CT findings for pulmonary tuberculosis before and after treatment to assess disease activity and efficacy of treatment.Methods：We retrospectively analyzed 52 cases of newly diagnosed tuberculosis proved by bacteriology thinsecting CT scans before and after treatment.Results：Micronodules,nodules,trees-in-bud sign,consolidation and cavities were the most common thin-secting CT findings were seen in active pulmonary tuberculosis.The disappearance of the micronodules,nodules and trees-in-bud sign,absorption of the consolidation,narrow of the thick-walled cavities or transformation into thinwalled cavities,the regression of pleural effusion and presence of the fibrotic change expressed the effectiveness of treatment.thin-secting CT can identify new tuberculous lesions from the fibrotic lesions.Conclusion：Thin-secting CT helps to determine activity of tuberculosis and validate the efficacy of antituberculous drugs.
