抗Clq抗体在评价狼疮性肾炎活动性中的作用 被引量:1

The role of anti- Clq antibodies in the evaluation of lupus nephritis activity

摘要 目的探讨抗clq抗体在评价狼疮性肾炎(LN)疾病总体活动性和肾脏活动性中的作用。方法选择2008年8月至2011年8月于北京大学深圳医院肾活检病理诊断为LN患者46例。ELISA方法检测血清抗Clq抗体滴度,比较该抗体与LN的活动性评分如SLEDAI评分、m—SLEDAI(modified SLEDAI)评分和肾脏活动性指数(RAS)等相关性。结果抗Clq抗体滴度与SLEDAI、m—SLEDAI呈正相关。当肾脏活动指数RAS〉11分时,抗Clq抗体(〉20u/m1)、抗dsDNA抗体(〉100u/m1)、补体c3(〈O.8mmol/L)、CRP(〉8mg/L)与对应的RAS值进行卡方检验,抗Clq抗体和抗dsDNA抗体差异有统计学意义(P〈0.05),皮尔逊卡方值分别为17.043,8.696,其列联系数分别为0.807、0.633。结论预测LN疾病整体活动性时,抗C1q抗体优于抗dsDNA抗体、补体C3;它与肾脏活动性指数有较好的对应关系。 Objectives To investigate the role of anti - C1 q antibodies in the evaluation of lupus nephritis activity including systemic activity and renal activity. Methods It was enrolled 46 lupus nephritis patients proved by renal biopsy in Peking University Shenzhen Hospital from August 2008 to August 2011. Serum titer of Anti - Clq antibody was detected by enzyme -linked immunosorbent assay (ELISA) following the manufacturer~ instructions. We conducted statistical analysis on Anti -C1 q antibody, which has correlation with SELENA -SLE Disease Activity Index and modified SELENA - SLE Disease Activity Index ( m - SLEDAI) or Renal Activity Score. Results There was a strong positive correlation between the presence of anti - Clq antibody and SLEDAI and m - SLEDAI. As renal activity score (RAS) 〉 11 with anti - C1 q antibody 〉 20ug/ml, anti -dsDNA antibodies 〉 100u/ml, C3 〈 0.8mmol/L, CRP 〉 8mg/L, Pearson chi - square test was accomplished between RAS and above parameters. Statis- tical significance was only found in anti -Clq antibody and anti - dsDNA antibody with RAS. Pearson chi -square values and Contingency coefficient of anti - Clq antibody and anti - dsDNA antibody were 17. 043, 0. 807and 8. 696, 0. 633, respectively. Conclusions Anti -Clq antibody had more predict value in systemic LN activity than anti -dsDNA antibodies and C3. It was correspondence with renal activity score.
