游离上臂外侧分叶皮瓣在手部多创面皮肤缺损中的应用

The appficalion of lateral ann mulllfoliate flaps to repair multiple soft tissue defects of the hand

目的探讨游离上臂外侧分叶皮瓣修复手部多处深部组织外露创面的临床疗效。方法2009年10月至2011年12月，对15例同时患有手部2～3处皮肤软组织缺损伴骨、肌腱外露的患者，择期采用游离上臂外侧分叶皮瓣同时修复。其中手掌部皮肤缺损伴手指皮肤缺损6例，手背伴手指皮肤缺损4例，手掌伴手背皮肤缺损3例，2指皮肤缺损2例。结果术后随访时间6～24个月，15例皮瓣全部存活，皮瓣质地优良，外形与手功能恢复满意。结论利用桡侧副动脉皮穿支设计的上臂外侧皮瓣血管解剖恒定，血管蒂长，是修复手部多处皮肤缺损的有效方法。

Objective To explore the application of lateral ann mulfifoliate flaps based on the radial collateral artery to repair multiple deep tissue defects of the hand and report its clinical outcomes. Methods From October 2009 to December 2011, 15 cases of more than one soft tissue defects over the hand with tendon or bone exposure were treated with free lateral ann multlfoliate flaps. There were 6 cases with volar hand skin defect and finger skin defect, 4 cases with dorsal hand defect and finger skin defect, 3 cases with both volar and dorsal hand skin defects, and 2 cases with skin defects of more than one finger. Free multifoliate flaps based on the radial collateral artery were used to repair those complicated skin and soft tissue defects. Results All the cases were follow-up for 6 to 24 months. All 15 flaps survived with satisfactory appearance and good hand function. Conclusion Transfer of lateral arm muhifoliate flaps based on radial collateral artery is an effective method to repair more than one skin and soft tissue defects of the hand.
