不同胃黏膜病变中Runx3表达及其与幽门螺杆菌感染关系的临床研究

摘要 背景:Runt相关转录因子3(Runx3)是近年新发现的肿瘤抑制基因,幽门螺杆菌(Hp)感染可能导致Runx3蛋白表达下调,进而参与胃癌的发生。目的:研究不同胃黏膜病变中Runx3表达及其与Hp感染的关系。方法:选取2010年3月~2011年3月广州军区广州总医院40例慢性萎缩性胃窦炎、40例胃窦溃疡和40例胃窦癌患者。应用13C-尿素呼气试验和Giemsa染色检测Hp感染情况,免疫组化SP法测定Runx3蛋白表达,并分析其与Hp感染的关系。结果:慢性萎缩性胃窦炎组、胃窦溃疡组和胃窦癌组的Hp阳性率(分别为45.0%、82.5%、57.5%)差异有统计学意义(P<0.05);Runx3蛋白表达呈逐步下降趋势(蛋白阳性率分别为70.0%、62.5%、35.0%),差异有统计学意义(P<0.05)。Hp阳性率与Runx3蛋白表达呈负相关(P<0.05)。结论:在胃黏膜癌变过程中,Hp感染和Runx3蛋白表达缺失可能是胃癌的危险因素。 Background: Runt-related transcription factor 3 (Runx3) is a novel tumor suppressor gene discovered in recent years, and Helicobacter pylori (Hp) could down-regulate the expression of Runx3 protein, thereby involving in the pathogenesis of gastric cancer. Aims: To investigate the expression of Runx3 in different gastric mucosal lesions and its relationship with Hp infection. Methods: Forty patients with chronic atrophic antral gastritis, 40 gastric antral ulcer and 40 gastric antral carcinoma from March 2010 to March 2011 at General Hospital of Guangzhou Military Command of PLA were enrolled. Hp infection was detected by 13 C-urea breath test and Giemsa stain. Immunohistochemical SP staining was used to detect the expression of Runx3 protein, and its relationship with Hp infection was analyzed. Results: Significant difference in Hp infection rate was found among chronic atrophic antral gastritis group, gastric antral ulcer group and gastric antral carcinoma group ( positivity rates : 45.0%, 82.5% and 57.5%, respectively) ( P 〈 0.05 ). Runx3 protein expression rate was gradually decreased in the evolution of gastric mucosal lesions (positivity rates: 70.0%, 62.5% and 35.0%, respectively) (P 〈 0.05). Hp infection rate was negatively correlated with expression of Runx3 protein (P 〈 0.05 ). Conclusions: In the process of development of gastric mucosa carcinogenesis, Hp infection and low expression of Rnnx3 protein may be the risk factors of gastric cancer.
