颈-股脉搏波速度与居家老人血清高分子量脂联素的相关性 被引量:5

Correlation of the Carotid-Femoral Pulse Wave Velocity with Plasma High Molecular Weight-Adiponectin Level of Dewelling Persons

摘要 目的探讨颈-股脉搏波速度(cfPWV)与居家老人血清高分子量脂联素的相关性。方法选择87例体检中心进行健康体检的居家老人。受试者的临床资料包括年龄、性别、腰围、臀围、收缩压、舒张压、肝肾功能、高密度脂蛋白、低密度脂蛋白、甘油三酯、总胆固醇、空腹血糖、餐后2 h血糖、空腹C肽、空腹胰岛素、血清总脂联素及高分子量脂联素、cfPWV。同时计算体质指数。按照cfPWV=9 m/s为界分为两组,cfPWV<9 m/s组(21例)和cfPWV≥9m/s组(66例)。结果两组间体质指数、收缩压、舒张压、肌酐、高密度脂蛋白、低密度脂蛋白、甘油三酯、总胆固醇、血清总脂联素水平及高分子量脂联素水平差异有统计学意义(P<0.05),但性别、年龄、总蛋白、球蛋白、白蛋白、总胆红素、结合胆红素、谷草转氨酶、谷丙转氨酶、尿素氮、尿酸、空腹血糖、餐后2 h血糖、空腹C肽、空腹胰岛素及胰岛素抵抗指数(HOMA-IR)差异无统计学意义(P>0.05)。多元回归分析显示血清高分子量脂联素、总脂联素、高密度脂蛋白与cfPWV呈负相关(OR^=0.011,0.019,0.125;P=0.011,0.012,0.015),收缩压与cfPWV呈正相关(OR^=1.046,P=0.045)。结论血清高分子量脂联素和总脂联素高水平可能是动脉硬化保护因素。 Aim To determine the correlation of the carotid-femoral pulse wave velocity (cfPWV) with plasma high molecular weight-adiponectin (HMW-ADP) level of dewelling persons. Methods 87 dewelling persons who un- derwent health examination in medical examination center were surveyed in this experiment. The clinical data of subjects contained age, gender, waistline, systolic blood pressure ( SBP), diastolic blood pressure ( DBP), hepatic function, renal function, high density lipoprotein cholesterol (HDLC) , low density lipoprotein cholesterol (LDLC) , triglyceride (TG), total cholesterol (TC) , fasting blood glucose (FBG), postprandial 2 hour plasma glucose (2hPG), fasting C-peptide, plasma total adiponectin level, plasma HMW-ADP level, cfPWV, HOMA-IR,and body mass index (BMI). All subjects were divided into two groups in which group A contained 21 cases (cfPWV 〈 9 m/s) and group B contained 66 cases (cf- PWV≥9 m/s). Results The difference of BMI, SBP, DBP, the serum levels of HDLC, LDLC, TG, TC, total- ADP, and HMW-ADP between the two groups had statistical significance (P 〈0. 05 ), however, no significant differences occurred in age, gender, total protein, globulin, albumin, total bilirubin (TBIL) , conjugated bilirubin ( CB ), aspartate aminotransferase(AST) , alanine aminotransferase(ALT) , blood urea nitrogen (BUN) , serum creatinine (Scr) , serum uric acid ( UA), fasting blood glucose ( FBG), postprandial 2 hour plasma glucose ( 2hPG), fasting C-peptide, fastinginsnlin (FIN) and homeostasis model assessment of insulin resistance (HOMA-IR) ( P 〉 0. 05 ). Multivariate regression analysis revealed that the serum levels of HMW-ADP, total-ADP, and HDLC were negatively correlated with cfPWV ( OR = 0. 011, 0. 019, 0. 125 ;P = 0.011, 0. O12, 0. 015 ), while SBP was positively correlated with cfPWV (OR = 1. 046, P = 0. 045 ). Conclusions High levels of HMW-ADP and total-ADP in serum may be the protective factors for angiosclerosis.
