以缺血为首发症状的脑动脉瘤的诊断与治疗

Diagnosis and treatment of intracranial aneurysm presenting with cerebral ischemia

目的探讨以脑缺血发作为首发症状的颅内动脉瘤的诊断与治疗。方法回顾性分析4例以脑缺血发作为首发症状的颅内动脉瘤的临床资料。1例行动脉瘤夹闭并载瘤动脉重建术,2例行动脉瘤孤立术,1例行支架辅助弹簧圈栓塞治疗。结果 4例均是血栓性巨大动脉瘤。术后4例患者脑缺血症状均得到明显改善。结论以脑缺血为首发症状的颅内动脉瘤都是血栓性巨大动脉瘤,应在术前制定个性化治疗方案,积极进行外科手术或栓塞治疗,可明显改善脑缺血的发作,避免动脉瘤破裂。

Objective To explore the clinical characteristics and surgical management of intracranial aneurysms presenting with cerebral ischemia stroke. Methods The clinical data of 4 patients with intracranial aneurysms presenting with cerebral ischemia stroke, of whom, 1 underwent clipping of the aneurysms and reconstruction of the parent artery, 2 trapping of the aneurysms and 1 sent-assisted coils embolization, were analyzed retrospectively. Results Four aneurysms were giant and thrombotic ones. The cerebral ischemia symptoms were relieved in all the patients after the treatment. Conclusions The intracranial aneurysms presenting with cerebral ischemia stoke usually are giant and thrombotic ones. The cerebral ischemia symptoms may be relieved and aneurysm rupture may he prevented by surgical occlusion or endovascular embolization of the aneurysms.
