胃癌患者基质金属蛋白酶组织抑制因子2的表达与其临床检测的意义 被引量:1

摘要 目的探讨在正常胃黏膜组织、胃癌组织及胃区域淋巴结中基质金属蛋白酶组织抑制因子2(TIMP-2)的表达情况。方法选取经病理证实的胃癌标本96例,胃癌区域淋巴结96例,正常胃黏膜60例,全部病例均随访5年以上。采用免疫组织化学方法的链霉菌抗生物素蛋白-过氧化物酶连接(SP)法检测上述标本中TIMP-2的表达。结果各病理因素TIMP-2表达阳性率:高中度分化与低分化69.0%vs 18.4%(P<0.01);浸润深度T1+T2与T3+T465.9%vs 36.4%(P<0.01);临床分期Ⅰ~Ⅱ型比Ⅲ~Ⅳ型71.1%vs 34.5%(P<0.01);无淋巴结转移与有淋巴结转移64.3%vs 37.0%(P<0.01);生存期≥5年与<5年73.0%vs 33.9%(P<0.01)。结论胃癌组织中TIMP-2表达的下降在胃癌的侵袭转移及预后中发挥着重要的作用。TIMP-2的表达对预测胃癌侵袭转移、评估患者预后状况及合理指导胃癌的综合治疗等具有重要的临床意义。 Objective To explore the expression oi nssue matrix metalloproteinase inhibitor-2(TIMP-2) in normal gastric mucosa tissues, human gastric cancer tissues and gastric regional lymph nodes. Methods The study involved pathologically confirmed gastric cancer specimens in 96 cases, gastric cancer regional lymph nodes in 96 cases, 60 cases of normal gastric mucosa, all patients were followed up for more than five years. Streptomyces avidinperoxidase(SP) method was used to detect the expression of TIMP-2 in the specimens. Results TIMP-2 expression positive rate in various pathological factors : highly, moderately differentiated and poorly differentiated 69.0 % vs 18.4 % (P〈0.01);depth of invasion T1+T2 and T3+T465.9% vs 36.4%(P〈0.01) ;clinical stage Ⅰ-Ⅱ type Ⅲ-Ⅳ 71.1% vs 34.5%( P d0.01) ;without lymph node metastasis and with lymph node metastasis 64.3% vs 37.0%( P 0.01) ;survival of ≥ 5 years and 〈 5 years 73.0% vs 33.9 % ( P 〈0.01 ). Conclusion The decreased expression of TIMP-2 in gastric cancer plays an important role in gastric cancer invasion metastasis and the prognosis of gastric cancer patients. To predict invasion metastasis, assess patients' prognosis and reasonably guide comprehensive treatment of gastric cancer,clinical detection of gastric carcinoma and regional lymph node tissue TIMP-2 expression has an important clinical significance.
