摘要
目的 :探讨亚临床甲状腺功能减退对亚临床甲状腺功能减退模型大鼠心肌组织结构的影响。方法 :采用"甲状腺全切术+术后皮下注射L-T4"法制备亚临床甲减大鼠模型,设定空白对照组、模型组、中药低剂量组、西药组、中+西药组及中药高剂量组,ELISA法按照试剂盒说明测定心肌羟脯氨酸及胶原蛋白含量,并切取部分心肌组织制作切片做光镜下与电镜下观察,同时观察温肾方对其指标的干预情况。结果 :与空白对照组比较,模型组大鼠心肌羟脯氨酸、胶原蛋白含量降低(P<0.01);与模型组比较,西药组、中+西药组和中药高剂量组心肌羟脯氨酸、胶原蛋白含量明显增高(P<0.01);中药高剂量组心肌羟脯氨酸及胶原蛋白水平接近于空白对照组(P>0.05)。模型组大鼠心肌组织在×400放大后,HE染色后显示排列紊乱,部分细胞肿胀明显;电镜下心肌细胞核不规则,染色质边集,胞质中线粒体明显肿胀,线粒体嵴损伤、断裂,形成不规则的增大的间隙,线粒体基质溶解,肌质网肿胀,可见肌节,但肌节中各带不明显;温肾方能在一定程度上逆转这些病理变化。结论 :亚临床甲状腺功能减退引起心肌胶原蛋白含量及心肌组织结构的改变,温肾方能逆转这些变化,从而具有防治亚临床甲减心脏并发症的作用,为中药治疗亚临床甲减防治并发症提供实验依据,温阳补虚法可能为有效治疗亚临床甲减的方法之一。
Objective:This topic, based on the old theory of traditional Chinese medicine and professor CHEN's clinical experience, was to observe the myocardial tissue pathological changes of SCH by experimental SCH rats. Methods : 48 experimental SCH Wistar rats, made by reseeted thyroid tissue and replaced therapy of L-T4, were divided into 6 groups: the blank control group, the model group, the group treated by low-dose lVenshenfang everyday, the group treated by high-dose Wenshenfang everyday, the group treated by L-T4 everyday and the group treated by L-T4 and Wenshenfang everyday. The cardiomyocyte apoptosis and the pathomorphological changes of heart were researched. The effects on the index of Wenshenfang were observed as well. Results : It was found that compared with the blank control group, the content of myocardial hydroxyproline and collagen reduced ( P〈0.01 ); the group treated by L-T4 and IVenshenfang everyday and the group treated by high-dose Wenshenfang everyday had raised ( P〈0.01 ) compared with the model group, and the content of the group treated by high-dose Venshen[ang everyday was approximately the same as that of the blank control group ( P〉0.05 ). The rats' myocardial tissue after HE-stained was enlarged 400 times through microscope, we inspected that the cells of the group treated by L-T4 everyday, the group treated by L-T4 and Wenshenfang everyday and the group treated by high-dose lVenshenfang everyday were approximately the same as that of the blank control group. We also inspected that the model group rats cardiac tissue muscle nuclei was irregular. The myocardial tissues of rats in the group treated by L-T4 everyday, the group treated by L-T4 and Wenshenfang everyday and the group treated by high-dose Wenshenfang everyday were with irregular nuclei, perinuclear space locally increasing, cytokinesis in myofihrillar arranging well, obvious sareomere, mitochondria swollen, sarcoplasmic reticulum mild dilatation. Conclusion : The present experiments indicate that SCH is associated with the variation of the content of myocardial collagen, structure and function of myocardium in rats. lVenshenfang could reverse these changes to a certain degree. This topic is expected to afford the theory evidence for treating SCH and advance the theory of treating SCH by warming Yang and tonifying kidney therapy.
