胸腰段陈旧性椎体骨折椎管狭窄的术式探讨

Discussion on the operative technique of old corpus vertebrae fracture

目的 :探讨用自行设计的经后路椎体横断截骨矫正术治疗陈旧性胸腰椎骨折的疗效。方法 :后路正中切口 ,咬除骨折椎一侧或双侧横突 ,切除一侧椎弓根。凿除骨折椎后凸部分椎体 ,直视下从椎体腰部横行切断 ,用RF钉复位固定、植骨融合 ,临床应用 2 0例。结果 :术后椎体畸形完全矫正 ,脊神经均有不同程度恢复。结论 :该手术难度不大 ,损伤小 ,椎体、脊柱畸形矫正满意 。

Objective:To discuss the effects of the treatment of old corpus vertebrae fracture with an self-designed operative technique,across the medial vertebrae,through the posterior approach.Methods:A longitudinal incision was made over the spinous processes,the unilateral or bilateral was cut off cross the process of the fracture vertebrae,the projection was carved behind it ,the medial vertebrae was severed crossly,the deformity in spine was corrected by RF,and the cross-sections were fused.Twenty cases were treated with the technique.Results:The deformity was corrected,and the nerve function was improved.Conclusions:The technique is not difficult,but only shows a little impairment.The deformity is corrected satisfactorily.Few stability and function of spine are damaged.
