综合护理法对老年痴呆患者动态血压节律的影响

Effects of Comprehensive nursing process to Circadian Rhythm of ambulatory Blood Pressure on senile dementia

目的:探讨综合护理法对老年痴呆并高血压病患者动态血压昼夜节律的影响。方法:对2008年6月至2010年12月老年痴呆合并高血压60例患者随机分成2组,研究组施行综合护理法护理患者(在施行一般内科护理同时加强病房管理,并结合图片、实物、多媒体、健康教育等方法指导患者服药,进行肢体运动功能训练、帮助患者恢复及强化记忆);对照组采用老年内科患者常规护理。以上2组患者,均记录护理干预前后动态血压昼夜节律变化情况、采用简易精神状态检查量表(MMSE)、日常生活活动量表(ADL)对患者的精神状态、遵护嘱行为等进行评定,以确定临床护理效果。结果:研究组患者实施综合护理方法干预后MMSE及ADL评分、遵护嘱行为与对照组相比有显著性差异(P<0.05)。研究组的夜间平均收缩压、平均舒张压及日间平均收缩压、平均舒张压均低于对照组及干预前(P<0.05),有显著性差异。结论:综合护理方法的综合性、个体化,对AD患者的动态血压昼夜节律失衡有明显改善作用且能明显提高AD患者的生活质量。

Objective ：To explore the influence of Comprehensive nursing process to the Circadian Rhythm of ambulatory Blood Pressure on senile dementia. Methods ：60 patients with senile dementia were apart to two groups randomly. One group patients received Comprehensive nursing process （ except routine nursing process, combining the management of the patient ward, pictures, health educa- tion, entity, media to help the patients restore and reinforce the memory and direct dose and limbs exercise）. The control group patients received routine nursing process. Two groups, circadian rhythm of blood pressure were all recorded after nursing process intervene, All patients were evaluated with the MMSE, ADL about mental state, obey the nursing order. Results： For the study group, the night average systolic pressure , average diastole pressure and the day average systolic pressure, average diastole pressure were lower than the control group, have significant difference （ P 〈 0.05 ）. Conclusions： The comprehensive nursing process can improve the disorder circadian rhythm of ambulatory blood pressure with senile dementia , can improve the quality of their life.
