78例急性结石性胆囊炎行腹腔镜胆囊切除术的临床疗效

Clinical efficacy of laparoscopic cholecystectomy for 78 cases of acute cholecystitis

目的探讨腹腔镜胆囊切除术治疗急性结石性胆囊炎的临床疗效。方法回顾性分析2009年2月～2010年10月78例行腹腔镜胆囊切除术的急性结石性胆囊炎患者的临床资料,并与同期80例急性结石性胆囊炎行开腹胆囊切除术患者进行对照,统计并比较两组患者手术时间、平均住院时间、胃肠功能恢复时间以及并发症发生情况。结果腹腔镜组手术时间平均为(72.1±12.7)min,开腹手术组为(56.4±11.0)min,差异有统计学意义(P<0.05);腹腔镜组平均住院时间为(6.6±1.9)d,明显短于开腹手术组的(9.8±2.7)d,差异有统计学意义(P<0.05)。腹腔镜组排便、排气、肠鸣音、进食时间均短于开腹手术组(P均<0.05),并发症发生率低于对照组(P<0.05)。结论腹腔镜胆囊切除治疗急性结石性胆囊炎微创,并发症少,胃肠功能恢复快,住院时间短,但手术时间相对较长,对医生的技术要求较高。

Objective To investigate the clinical efficacy of laparoscopic cholecystectomy for acute cholecystitis. Meth- ods Retrospective analysis clinical data of 78 patients with acute eholeeysliTis who suffered from laparoscopic choleeysteetomy from February 2009 to October 2010 was reviewed, and 80 cases who had taken surgical operation at the same time were adopled into the study as control. Operation time, average length of stay, recovery time of gastrointestinal function and complications were statistically compared. Results operative time was （72. t ± 12. 7） rain in laparoscopic group and （56.4 ± 11. 0 ） rain in surgery group, with statistically significant difference （ P 〈 0. 05 ） ; average length of stay was （ 6. 6 ± I. 9 ） d in the laparoscopic group, which was significantly shorter than that in surgery group（.9.8 ± 2. 7 ） d （P 〈 0. 05 ）. Laparoscopic bowel movement ,exhaust ,bow- el sounds ,feeding time are shorter than the open surgery group （P 〈0.05） ; complication rate was lower than the control group （P 〈 0. 05 ）. Conclusion Laparoscopie cholecystectomy has minimally invasive, fewer complications, faster recovery of gastroin- testinal function, shorter hospital stay, but the relatively long operation time, for acute cholecystitis, and the doctor＇s techniques are required.
