摘要
目的探讨前列腺液DD3 mRNA检测在前列腺癌早期诊断中的应用价值。方法收集前列腺癌患者、良性前列腺疾病患者和非前列腺疾病患者的前列腺液,采用RT-PCR方法对其中的DD3 mRNA表达进行研究,分析三组患者DD3 mRNA表达的差异,探讨DD3 mRNA表达与前列腺癌临床分期和病理分级的关系。同时用ROC曲线对DD3 mRNA诊断前列腺癌的效能进行分析。结果前列腺癌组DD3 mRNA表达量明显高于良性前列腺疾病组和非前列腺疾病组,差异具有显著性(P<0.05),而后两组之间差异无显著性;前列腺癌患者DD3 mRNA表达量与临床分期无关,但不同病理分级之间差异有显著性(P=0.017);DD3 mRNA的ROC曲线线下面积高于tPSA线下面积,当DD3 mRNA相对含量值取0.63时其诊断效能最高。结论前列腺液DD3 mRNA检测对于前列腺癌的诊断具有较高的灵敏度和特异性,作为前列腺癌的诊断指标具有较好的发展前景。
Objective To investigate the value of DD3 mRNA detection in prostatic fluid for the early diagnosis of prostate cancer.Methods Prostatic fluid was collected from patients with prostate cancer,benign prostate disease and non-prostate disease,RT-PCR was performed to detect DD3 mRNA expression,DD3 mRNA expression differences were analyzed in the three groups of patients,the relationship of DD3 mRNA expression in prostate cancer clinical stage and pathological grade was explored.The diagnosis performance of DD3 mRNA for prostate cancer was analyzed using ROC curve.Results DD3 mRNA expression level in prostate cancer group was significantly higher than that of benign prostate disease group and non-prostate disease group,the difference was significant(P0.05),and there was no significant difference between the latter two groups;There was no relationship between DD3 mRNA expression in prostate cancer patients with different clinical staging,but the differences between the different pathological grades was significant(P=0.017);the AUC of DD3 mRNA was higher than that of tPSA,and when DD3 mRNA relative content value was 0.63,there was the highest diagnostic efficacy for DD3 mRNA.Conclusion Prostatic fluid DD3 mRNA detection,with high sensitivity and specificity for the diagnosis of prostate cancer,has good prospects for development as a diagnostic indicator of prostate cancer.
