肺剂量体积直方图参数V5与放射性肺炎关系的研究 被引量:1

摘要 目的分析剂量体积直方图(DVH)参数V5与胸部肿瘤放射性肺炎(RIP)之间的关系。方法回顾性分析行了调强放疗的85例胸部肿瘤患者的临床资料,其中未限制V5(V5未限制组)共43例,限制V5(V5限制组)共42例,观察2组RIP的发生情况。结果根据放射治疗肿瘤协作组(RTOG)诊断标准,V5未限制组发生2级及以上RIP19例,发生率为44.2%,V5限制组发生2级及以上RIP12例,发生率为28.6%,2组相比差异无统计学意义(P〉0.05),但Spearman等级相关分析研究表明V5与RIP发生具有明显相关性(P〈0.01)。结论DVH参数V5与RIP发生具有明显相关性。 Objective To evaluate the relationship between the dose -volume histogram (DVH) parameter V5 (the volume of normal lung receiving 5 Gy) , and radiation induced pnenmonitis (RIP). Methods Eighty - five patients with thoracic malignancies treated with intensity -modulated radiotherapy were retrospectively reviewed. Forty -three patients received radiation therapy without restricting V5 ( V5 unrestricted group) and 42 patients were irradiated with constraint of V5 (V5 restricted group). The RIP was evaluated for both groups. Results Acording to the Radiatin Therapy Oncology Group (RTOG) criteria RIP, 12 patients (28.6%) in Vs unrestricted group and 19 patients (44.2%) in V5 restricted group developed ≥grade 2 RIP. There was no significant difference between these two groups (P 〉 0.05). However, Speaiman correlation analysis showed a remarkable correlation between V5 and RIP (P 〈 0.01 ). Conclusion There is association between the V5 and RIP.
