摘要
目的 探讨p15 ,血管内皮生长因子 (VEGF)与胃癌临床病理因素的关系。方法 应用免疫组织化学 (S P)方法测定胃癌及胃良性病变中p15 ,VEGF表达。结果 胃癌中p15阳性率 (4 3 75 % )低于胃良性病变 (6 9 2 3 % ) (P <0 .0 5 )。p15表达与临床病理因素无关。VEGF阳性率胃癌组为 75 0 0 % ,胃良性病变组为 7 6 9% (P <0 .0 0 1) ;胃癌中浸润深度不超过粘膜下层组为 42 86 % ,浸润肌层组为 5 1 72 % ,浸润浆膜层组为 95 45 % (P<0 .0 0 1) ;有淋巴结转移组为 82 76 % ,无淋巴结转移组为 5 4 5 6 % (P <0 .0 5 ) ;伴远处转移组为 10 0 % ,不伴远处转移组为 71 10 % (P <0 .0 5 ) ;pTNM分期Ⅰ ,Ⅱ组为 5 3 13 % ,Ⅲ ,Ⅳ组为 89 5 6 % (P <0 .0 0 1) ;溃疡组为 81 97% ,浸润型组为 40 0 0 % (P <0 .0 5 )。p15(- )与VEGF(+ )具有显著相关性 (P <0 .0 0 1)。p15 (- ) VEGF(+ )联合表达率胃癌组为42 5 0 % ,胃良性病变组为 7 6 9% (P <0 .0 1)。结论 胃癌中存在p15下调与VEGF上调 ,且二者具有相关性。p15表达与临床病理因素无关 。
Objective To investigate the significance and relationship of p15, vascular endothelial growth factor(VEGF) with gastric carcinoma(GC). Methods Using immunohistochemical staining to examine the expressions of p15 and VEGF in GC and gastric benign disease(GBD). Results The positive expression rate(PER) of p15 was significantly lower in GC than that in GBD(43.75% vs 69.23%, P<0.05). No relationship was found between PER of p15 and clinicopathologic factors. The PER of VEGF in GC was 75.00%, in GBD 7.69%(P<0.001). The PER of VEGF in ulcerative type and infiltrating type of GC was 81.97% and 40.00% respectively(P<0.05). PER of VEGF in GC with serosa invasived was 95.45%, in GC with muscle invasived 51.72%(P<0.001), in GC with mucosa or submucosa invasived 42.86%(P<0.001). PER of VEGF in GC with lymph node metastasis was82.8%, without lymph node metastasis 54.6%(P<0.05). PER of VEGF in GC accompanied by distant metastasis(100%) was higher than that without distant metastasis(71.1%,P<0.05). PER of VEGF in pTNMⅠ and Ⅱ was 53.13%, in Ⅲ and Ⅳ 89.56%(P<0.001). Negative expression of p15 correlated significantly with positive expression of VEGF(P<0.001). Conclusions p15 expression down regulation has relationship with GC, but no relationship with clinicopathologic factors. VEGF expression shows significant relations with clinicopathologic factors. There is a significant relationship between p15 expression down regulation and VEGF expression up regulation in this study.
