社区健康管理对老年高血压控制效果的研究

Efficacy of the follow-up visits on elderly with hypertension in a community health service center

目的评价对老年高血压患者实施社区健康管理的效果。方法选择安徽省合肥市瑶海区铜陵路街道社区已确诊的414例老年高血压患者(年龄≥65岁)进行健康管理2年,每3个月随访1次,进行体格检查并对生活方式进行指导。结果健康管理后,患者的收缩压、舒张压和体质指数均有显著降低。2年后女性的收缩压下降较男性更为明显,1级高血压患者的健康管理效果更显著。结论社区健康管理对于老年高血压的控制有明显效果。

Objective To find out the efficacy of follow - up visits on senior citizens with hypertension in Yaohai Tong ling Road community. Methods Altogether 414 incidents （ ≥65 years old） with high blood pressure underwent follow - up visits every 3 months for 2 years, including periodic health examination and lifestyle evaluation. Results There were drastic decreases in systolic blood pressure, diastolic blood pressure and body mass index. Moreover, women＇s systolic blood pressure descended more dramatically than that of men; also, patients categorized as grade 1 hypertension were found in an optimal reduction in their blood pressure. Conclusion Evidence showed that follow- up visits had an obvious value in controlling blood pressure among the old people.
